• Note (General): type:text; 【目的】わが国における患者移動介助技術は、患者の持ち上げや抱え上げが基本であり、1945 年に米国で提唱された「ボディメカニクス」によって行うことが看護基礎教育から現在も教示されており、いまだ看護師の腰痛有訴率は高い。しかし、現在の米国では2003 年に患者の移動介助技術に関する科学的根拠に基づくガイドライン「Safe Patient Handling and Movement（以下SPHM）」が開発され、看護師の筋骨格系障害の発症率が減少した。そこで、SPHM が導入されている米国で使用されている基礎看護学テキストにおける患者移動技術の内容について分析を行い、わが国の看護基礎教育に必要な患者移動介助技術について検討する基礎資料を得ることを本研究の目的とした。 【方法】Amazon.com International Around the World Unit StatesよりFundamentals of Nursingをキーワードに検出、5版以上改訂されている図書で最新版の教科書3件を抽出し、「ボディメカニクス」「安全な患者移動介助技術」に関する記述内容の検討を行った。 【結果・考察】ボディメカニクスの定義の記載は認めず「看護実践で歴史的に使用されていたボディメカニクスは、看護師やヘルスケアスタッフに身体損傷を引き起こす」ことが示されていた。わが国で使用されているすべての基礎看護学テキストは、いまなおボディメカニクス中心の患者移動技術が示されている。看護基礎教育の段階から人間工学に基づく適切な身体の動きを取り入れる必要があると考える。安全な患者移動介助技術は「人間工学の原則に基づいた安全な方法に置き換えること」「SPHM技術の活用は、患者の運動や介助、移動における最適な実践の基準」「人間工学を活用して操作される専用の介助機器」「lift teamの利用」が効果的なアプローチとして示された。米国においては、看護学校のカリキュラムにもSPHM の概念を導入し、看護学生に対しても、人間工学的な介助方法を効率的に習得、実践するための教育が行われていることが確認された。 【結論】米国の基礎看護学テキストには、ボディメカニクスの定義の記載はなく、人間工学に基づく身体の動きを取り入れること、SPHM技術の活用が患者移動介助技術の基準であることが示されていた。これにより、わが国の看護基礎教育に必要な患者移動介助技術について検討すべき事項が確認された。 Objectives: In Japan, the technique of assisting patient moving techniques essentially involves moving and lifting patients. Although nurses are presently taught to do so according to 'body mechanics,' as proposed in the United States (US) in 1945, the rate of lower back pain complaints remains high among nurses. However, in the US today, 'safe patient handling and movement (SPHM)' guidelines have been developed based on scientific grounds regarding techniques to assist in patient moving techniques. Such guidelines have reduced the present incidence of musculoskeletal disorders in nurses. Therefore, the purpose of the present study is to analyze content pertaining to techniques related to assisting with patient moving techniques. in basic nursing textbooks used in the US where SPHM has been introduced. This is done with the aim of obtaining basic data to examine techniques to assist in patient moving techniques, which are required in basic nursing education in Japan. Methods: Upon searching the Amazon.com website using the key words 'fundamentals of nursing,' we extracted three of the most recent textbooks among those books with five or more revised editions. We then examined the content descriptions regarding 'body mechanics,' and 'safe techniques to assist in patient moving techniques.' Results and discussion: We found no descriptions that defined body mechanics, but found that 'body mechanics that are historically used in nursing practice cause debilitating injuries to nurses as well as healthcare staff.' All basic nursing studies textbooks used in Japan still present techniques to assist in patient moving techniques that are based on body mechanics. It is thought that appropriate physical movement based on ergonomics should be incorporated during basic nursing education. It was found that, in safe techniques for assisting in patient moving techniques, effective approaches include 'shifting to safe methods based on ergonomic principles,''using SPHM techniques, which are the most suitable practice standards for patient movement, assistance, and repositioning,' 'using specialized assistance devices that are operated using ergonomics,' and 'using a lift team.' In the US, the concept of SPHM was introduced into nursing school curriculum, and it has been confirmed that nursing students are educated so as to effectively learn, and practice, ergonomic assistance methods. Conclusion: We found that basic nursing science textbooks in the US do not include a definition of body mechanics, but include physical movement based on ergonomics, and note that the use of SPHM techniques is the standard technique for assisting in patient moving techniques. Therefore, we confirmed that techniques required to assist in patient moving techniques are a matter that should be examined in basic nursing education in Japan.
