家庭と専門家とが連携し、家庭内で高い頻度で支援を実施する方法に遠隔地発達支援(以下、telehealth)がある。Vismara ら（2013）の先駆的な研究では、自閉スペクトラム症のある幼児を対象に、保護者による早期発達支援をtelehealthにて実施した。その結果、標的行動である、保護者の発達支援スキルが向上し、子どもの適切なコミュニケーション行動も増加した。このようなtelehealthによる支援は、応用的な観点から、子どもと保護者が通所施設に通うコストを低減し、同時にコロナ禍での新しい発達支援方法を提案するという点でも、重要であると考える。本研究では、5名の発達障害児（主に小学校1年生）とその保護者を対象に、小学校場面で遭遇することが想定される社会スキルが必要な現実的な状況を設定し、その状況下における社会行動、言語行動などの広い意味でのコミュニケーション行動の獲得を支援するためのプログラムを作成した。支援者は支援のためのプログラムと課題を開発し、それをオンラインで評定と再実施のための仕組みをつくった。支援プログラムと教材を研究者が、クラウドにアップロードし、保護者がそれをダウンロードして、家庭で実施した。支援者は、週１回オンラインにてその効果をプローブとして評定した。新たに開発したオンライン発達行動支援の効果を検証すること、及び参加児と保護者の満足度と負担度の評定を実施した。その結果、幅広い社会コミュニケーション行動が習得され、維持も認められた。保護者の満足度が高く、負担度の低いオンライン発達行動支援であることが示された。この研究の結果から、子どもの社会コミュニケーションスキルに効果的で、同時に保護者の満足度も高く、負担度が低いtelehealthプログラムの条件が示された。 The purpose of this study was to investigate the effect of a telehealth program to teach social communication skills to elementary students with developmental disorder. An online developmental and behavioral intervention program was newly developed. All tasks were assigned to the parents via an internet-enabled tablet PC. We conducted a semi-structured interview with the parent to determine the target social communication skills. The program materials included social situations in which the students often encountered at school. The researcher uploaded the materials at Cloud, the parents downloaded them and taught the student using them. The researcher evaluated the home-based teaching as the probe session by online meeting system Zoom once a week. Multiple baseline across tasks design was used. After completing the program, the correct social communication rates increased and the correct response maintained at one-month follow up. The parent reported the satisfaction of the program. These results suggested the effect of telehealth program for promoting students' social communication skills.
