• Note (General): type:text; 本課題は、コロナ禍において、認知症患者の意思を介護者がどのように把握し、尊重しようとするのかを、社会学的に研究するものである。 日本は現在、「新しい認知症ケア」の理念下にある。新しい認知症ケアとは、「認知症患者本人の思いやその人らしさを、できるだけ尊重すべきだ」という介護の考え方を指す。こうした理念のもとでは、介護家族と多領域からなる専門職の連携が重要となる。 ところがコロナ禍では、人との接触を削減することが求められる。多くの介護施設では、介護家族の面会を中止するなどの対応が見られた。新しい認知症ケアとコロナ禍対応は、両立が極めて難しい関係にある。 そうした困難な状況で、介護者はどのように連携し、認知症患者の意思を尊重しようとしているのか？ そこで本研究は、介護施設におけるコロナ禍での「リモート面会」の調査を実施した。これは、タブレット端末などを用い、認知症患者の居室と面会者（家族）用スペースをビデオ会議システムで結ぶ試みだ。 調査には、介護家族I氏（60代・女性）の協力を得た。I氏には、認知症を患い、介護施設Xに入所している実母K氏（90代）がいる。2020年末、施設Xがリモート面会を導入して以降、継続的な調査を実施した。 調査では、以下の点が明らかになった。まず、介護家族が患者の人生（ライフヒストリー）を参照し、面会を構成している点。15分程度に限られた面会時間のなかで適切にはたらきかけるため、家族は患者に馴染みのある話題（季節の花や歌等）を選んでいた。しかし、準備を重ねても、技術の不確定性が問題となる。X内のWi-Fi通信環境はしばしば不安定になり、タブレット端末は高齢者の発言を正確に収音するには機能が不足していた。そこでI氏と職員は、洗濯物の受け渡しの機会など、インフォーマルなやり取りから、互いに情報を補っていた。 コロナ禍でも、患者本人を尊重しようという試みは、このように継続していた。 The purpose of this study was to illustrate how Japanese caregivers have respected the personhood of dementia patients during the spread of COVID-19. In Japan, the person-centered care paradigm (Kitwood 1997) was accepted gradually between the 1980s and 2000s. Maintaining each dementia patient's personhood is regarded as the prime task in caregiving by many medical and healthcare professionals and family caregivers. However, as COVID-19 spread, reducing social contact with people became a requirement. For example, many long-term care facilities canceled family visits with patients. Respecting dementia patients and adapting to the new normal are extremely difficult to balance. In such difficult situations, how are caregivers working together to respect and maintain the personhood of people with dementia? In this study, I conducted a survey of "virtual visits" at long-term care facility X in an attempt to connect the room of a dementia patient and the space for visitors (family members) with a video conferencing system using a tablet device. In this study, I focused on the case of Ms. I and Ms. K in my detailed analysis. Ms. K suffers from dementia and is currently residing in facility X. Ms. I is her daughter. Their case is one of the first involving virtual visits at facility X. The major findings of my research follow. First, the family caregiver referred to the patient's life history and identified the topic of the visit. As visiting time was limited to 15 minutes, it was necessary to select a topic that was familiar to the patient. However, no matter how well the family prepared, technical problems remained. The Wi-Fi communication environment in facility X was often unstable, and tablet devices lacked the ability to accurately pick up the patient's remarks. Therefore, Ms. I and members of the nursing staff supplemented each other's information through informal exchanges, such as those that occurred when handing over the laundry. Even amid the COVID-19 crisis, caregivers attempt to respect and maintain the personhood of people with dementia through interactions.
