• Note (General): type:text; 大腸がん（colorectal cancer; CRC）の発生、死亡が過去数十年間、本邦では増加している。CRC の発生要因として生活習慣、メタボリック因子との関連が示唆されているが、本邦での両者の関連を示す大規模な報告はない。そこで今回の検討では、CRC の前癌病変と考えられている腺腫性病変の発生に対するリスク因子について、人間ドックにおける大規模な総合的な健診データを用いて検討を行った。 当院予防医療センターで2012 年8 月から2016 年12 月の間に総合的な人間ドック健診を行った10138 名の受診者中、大腸内視鏡検査を受けた者が2832 名、そのうち胸部CT を受検しなかった者を除外した2769 名を対象とした。年齢、性別、身体計測（体格指数（BMI））、CT により計測した内臓脂肪面積（VFA）、血圧、血液生化学的データ（脂質代謝、糖代謝）、高血圧、脂質異常症、糖尿病の治療歴、さらに飲酒・喫煙習慣を独立変数として検討を行った。大腸腺腫性病変（腺癌を含む）と各変数との関連を単変量解析により評価し、さらに多重ロジスティック回帰モデルを用いた多変量解析により検討を行った。 対象2769 例のうち327 例（11.8％）に大腸腺腫性病変（CRN）を認めた。また、676 例（24.4％）がMets に該当しており、Mets とCRN は有意に相関していた（P<0.01）。単変量解析では性別、年齢、BMI、VFA、収縮期血圧、LDL コレステロール、中性脂肪、空腹時血糖、HbA1c、インスリン、尿酸、脂肪肝、喫煙、飲酒、体重増加がCRN と有意な関連性を認めた。ロジスティック回帰分析では、年齢、喫煙、飲酒、内臓脂肪蓄積、BMI、収縮期血圧、LDL コレステロール、中性脂肪が独立したリスク因子であった。 今回の検討により、加齢、メタボリック要因、喫煙・飲酒習慣が大腸腫瘍発生②対するリスク因子である可能性が明らかとなった。今後、さらにアディポサイトカインの関与を追究していく。 In Japan for the past decades, the incidence and mortality rate of colorectal cancer (CRC) has been increased. The association of metabolic syndrome (MetS) with CRC has been reported in a number of studies, however, individual factors contributing to development of CRC have not been clear yet, especially in Japan. In this study, we have investigated the risk factors, especially focusing on MetS for development of colorectal adenomatous neoplasia (CRN) as precancerous lesions by using large scale comprehensive health checkup data. Among 10138 subjects who took comprehensive health checkup at our hospital between in August 2012 and December 2016, 2769 subjects who also underwent screening colonoscopy were analyzed. Demographic characteristics, anthropometric measurements, visceral fat area (VFA), blood pressure (BP) and hematological metabolic parameters as well as smoking and drinking habits were assessed. Association between variables and CRN was evaluated by univariate analysis, and then by multivariate analysis using multiple logistic regression model. A p value < 0.05 was considered statistically significant. Of 2769 subjects analyzed, 327 had CRN（11.8%) and 676 were diagnosed as MetS (24.4%) The presence of MetS was significantly associated with CRN (p < 0.01). Univariate analysis identified significant association of sex, age, body mass index (BMI), and VFA etc. with prevalence of CRN. Logistic regression analysis revealed that age, VFA, BMI, systolic BP, LDL, TG and smoking and drinking habits were independent factors associated with CRN. The results of his study suggested that aging, metabolic factors and smoking/drinking might be risk factors for colorectal neoplasia in Japan.
