• Note (General): type:text; 我々の研究グループは、2019年に発表した思春期特発性側弯症 (AIS)の発症のゲノムワイド相関解析(GWAS)のデータを基に、polygenic risk score (PRS)という遺伝統計学の手法でゲノム情報からAISの発症の予測モデル(AIS-PRS)を作成した。AIS-PRSで最もリスクが高い群に分類さる個人は、平均なリスク群に比べて約4倍AISを発症する可能性が高いことがわかった。また、AIS-PRCのarea under the receiver operating characteristic (AUROC)は、0.65であった。次に、AISのGWASの結果から、より重要なsingle nucleotide polymorphism (SNP)を選定する方法を作成した。そして、PRSを作成し、その予測能を評価した。すると、従来の約1/8のSNP数まで削減することができ、AUROCは0.68と同等以上の予測能を示した。AISにとって重要なSNPを抽出することで、さらに予測能を向上させることができる可能性を示した。さらに、疫学研究でAISの患者は健常人と比較してbody mass index (BMI)が低値の傾向があるため、はゲノム情報から作成したPRSに、BMIの値を説明変数として組み込むと、モデルのAUROCは0.71まで改善した。上記から、AISの環境的因子も同定していくことでより良い予測モデルを作成できる可能性が示唆された。続いて、我々はAISの重症化の予測モデルの作成を試みた。まず、GWASに使用した3つのコホートのコブ角測定の再現性を級内相関係数で評価した。3コホートともに非常に高い相関を示した。また、コブ角のGWASとAISのGWAS間に中等度のgenetic correlationを認め、AIS-PRSとコブ角の実測値の間にも有意な相関を認めた。そのため、AIS GWASのデータを用いて、重症化のPRSを作成した。重症化のPRSのAUROCは、0.69であり、妥当な結果を示した。一方で、AIS-PRS同様にBMIをモデルの説明変数に加えても、AUROCの改善は認めなかった。このことから、BMIは主にAISの発症に関与している可能性が示唆された。上記内容をまとめ、Journal of Bone and Mineral Research誌に投稿し、現在修正対応中である。今後は、今回作成したPRSの予測能の前向き研究や他人種での再現性の確認が課題である。 Based on the data from the genome-wide association study (GWAS) of adolescent idiopathic scoliosis (AIS) published in 2019, our research group developed a predictive model for the development of AIS (AIS-PRS) from genomic information using a genetic statistical method called polygenic risk score (PRS). Individuals in the highest risk group of the AIS-PRS were four times more likely to develop AIS than those in the average-risk group. The area under the receiver operating characteristic (AUROC) of the AIS-PRC was 0.65. Next, we developed a method to select more important single nucleotide polymorphisms (SNPs) from the results of the AIS GWAS. We then created a PRS and evaluated its predictive ability. The results showed that the number of SNPs could be reduced to about 1/8 of the previous method, and the AUROC was 0.68, which is equivalent to or better than the previous method. Furthermore, since AIS patients tend to have a lower body mass index (BMI) than healthy individuals in epidemiological studies, the AUROC of the model improved to 0.71 when the BMI value was included as an explanatory variable in the PRS generated from genomic information. These results suggest that better prediction models can be created by identifying the environmental factors of AIS. Next, we attempted to develop a prediction model for the severity of AIS. First, we assessed the reproducibility of Cobb angle measurements in the three cohorts used in the GWAS using intraclass correlation coefficients. A moderate genetic correlation was found between the Cobb angle GWAS and the AIS GWAS, and a significant correlation was also found between the AIS-PRS and the actual Cobb angle measurements. Therefore, we used the AIS GWAS data to create the PRS of severe disease. The AUROC of the PRS of severe disease was 0.69, indicating a reasonable result. On the other hand, as in the AIS-PRS, the addition of BMI as an explanatory variable in the model did not improve the AUROC. This suggests that BMI may be mainly involved in the development of AIS. The above information was summarized and submitted to the Journal of Bone and Mineral Research and is currently under revision. In the future, it is necessary to confirm the predictive ability of PRS in prospective studies and reproducibility in other races.
