• Note (General): type:text; 地域包括ケアシステム下での在宅医療移行が増える中、後方支援病院が発行する診療情報（診情書）には在宅医療機関の求める情報が含まれていない場合が多い。そこで本研究では特に複数疾患を持つ高齢者に関する診療情報の質向上を目的とし在宅医療経験のある医師に行った約40分間の半構造化面接をMAQqdaを用い帰納的に分析した（看護医療学部研究倫理委員会受理番号293）。 雪だるま式標本法により協力を得た家庭医療専門医20名（男性14名・女性6名）は卒後年数7-18年・在宅医療経験4-16年を有し、東京・神奈川・富山・福岡・千葉・長野・岐阜・兵庫に勤務していた。 分析の結果、在宅診療に必要な情報（薬・栄養・ADL）や死亡診断書・生命保険書類用の情報（診断日・手術日）の不備により、適切な医療が提供できない、時間が浪費される、減薬が困難、医療不信を招くなどの問題が生じた。病院医の多忙・心情、責任感の欠如、在宅医療の理解不足、院内の連携不足、経営上の要因、病院医・在宅医間の権力勾配、アピール不足がその理由とされ、義憤・諦観・理解・共感・発奮などの受け止め方がされていた。主治医・患者・家族・介護支援専門員・訪問看護師・地域連携室・過去の受診機関に聞く、初診面接・退院または自施設内カンファレンスを行うことに加え、共通書式を用いる、病棟看護師を在宅医療研修に受け入れる、研修医を教育するという取り組みがあった。結核の否定、患者人物像、再入院の可否、助言、連絡方法といった情報が望まれ、看護記録は診情書より有益な情報を含むが在宅医には入手困難、医師間の直接照会を最善としつつ回避する背景には、時間的損失、相手の体面への配慮、再入院を拒まれぬようにという実利、患者の元主治医への思いの尊重があると判明した。 医療圏の特性や組織文化、医師の力量も影響するという指摘から、今後は対象を広げ在宅医・病院医双方の視点を引き出すことが必要と思われる。 Background: One of the pressing issues under the current community-based comprehensive care system is inadequate information in the patient referral document provided by hospitalists to home care physicians. In order to explore issues surrounding the information gap in the referral document, especially for elderly patients with multimorbidity, semi-structured interviews were conducted with family physicians and inductively analyzed using the qualitative research software, MAQqda. Methods: Approximately 40-minute-long interviews were conducted with 20 certified family physicians (14 males and 6 females) with 7-18 years of clinical experience and 4-16 years of home care experience who are currently practicing in 8 different prefectures throughout Japan. Results: Six categories were extracted from the data: a) types of inadequate information (e.g., date of diagnosis / surgery), b) problems caused by the inadequate records (e.g., failure to provide adequate care), c) factors behind inadequate records (e.g., overwork, limited understanding of home care), d) measures taken to solve / prevent the problems (e.g., fulfilling the information gap through conversations with patients / family members), e) family physicians' reactions to inadequate records (e.g., indignation, resignation), and f) types of information family physicians would like to have included in the patient referral document (e.g., socio-psychological information about the patient). Conclusion: Interviewees repeatedly argued that the quality of the referral document greatly depends on the local customs, organizational culture, individual doctors' clinical competence, indicating that the further research should include both hospitalists and family physicians so that perspectives of both parties will be reflected in the future call for action toward adequate patient referral documents.
