本研究はCOVID-19 拡大下における精神看護専門看護師による看護師への心のケアの構造とプロセスを明らかにし、今後も起こりうる感染症パンデミックにむけた心のケアマニュアルを作成することを目的とするものである。今年度はマニュアル作成のための基礎資料を得ることを目的に以下の２つの課題に取り組んだ。1)COVID-19 に関連した医療者の心のケアに関連した国際機関や専門学会が提示するガイドラインや先行研究をレビューし、心のケア実践に求められる介入内容と手法を概観した。2)COVID-19 陽性患者を受け入れている病院に勤務する精神看護専門看護師17名を対象にインタビュー調査を行い、質的帰納的方法によりデータ分析を行った。データ分析の結果、精神看護専門看護師は、COVID-19陽性患者の受け入れに伴って組織全体が混乱する状況において、多職種と役割分担しながら感染症と闘う現場の最前線にいる看護師と組織との橋渡しを行うことで安心して仕事を継続できる環境を整えること、そして看護師の精神的不調を早期に発見し、精神的健康を回復、維持できるような情緒的、教育的を提供していること、そして感染予防対策により普段のケアができない葛藤状況において、看護師が患者ケアの自信を回復し、患者の精神的ケアに主体的に取り組めるような支援を提供することで、感染症パンデミック下における看護師・看護チームが直面している危機的状況への介入を行っていた。1)2)の結果をもとに、精神看護専門看護師によるアセスメントや介入内容、介入方法を記述した看護師支援のための心のケアマニュアルを作成にていく予定である。 Since the outbreak of coronavirus 2019(COVID-19) has occurred, nurses caring for patients at the frontline are at high risk of developing physical and mental health outcomes. This study aims to develop the clinical practice guidelines of psychological support for nurses provided by Certified Nurse Specialist in Psychiatric and Mental Health (CNS) during the pandemic. We worked on two tasks this year. First, we conducted literature review on psychological support and intervention for health care workers during COVID-19 Pandemic. Then, qualitative semi-structured interview data were collected from seventeen CNS to clarify their practice during COVID-19 Pandemic. We described the structure and process of psychological support provided by CNS for frontline nurses. And we are planning to develop the practice guidelines based on this result next year.
