• Note (General): type:text; 【目的】医薬品リスク管理計画（以下、RMP）は、医薬品の開発から市販後まで一貫したリスク管理をひとつにまとめた文書である。RMPにおけるリスク最小化活動は、医薬品を使用する患者の安全性の確保および改善を目的に行われる活動であるが、慶應義塾大学病院（以下、当院）の実態は不明である。そこで、当院の状況を把握するとともに、積極的にRMPを利活用するための方策を策定することで、リスク最小化に貢献することを目的とする。 【方法】当院採用医薬品において独立行政法人医薬品医療機器総合機構（PMDA）からRMPを作成している医薬品のデータを抽出し、服薬指導時に使用するテンプレートを作成した。また、当院薬剤部の病棟担当者を対象にRMPに対する意識ならびにRMPの利活用の実態を調査した。 【結果・考察】2019年2月～2020年4月に当院で採用となった薬剤について、RMPの3つの安全性検討事項を盛り込んだテンプレートを151品目について作成した。平行して、薬剤部の病棟担当者55人に対して、RMPに対する実態調査を行った。回答率は69.1％（38人/55人）であった。RMPを意識して病棟活動を行っているかとの質問には、意識しているが15人（39.5%）であった一方であまり意識していない病棟担当者が半分以上の23人（60.5%）いることが判明した。普段のRMPの使用については、良く使用している0人、使用している5人と低い一方で、あまり使用していない28人、全く使用していない5人と、普段あまり使用していないことも判明した。RMPを用いている2人から患者さんの治療に活かせたと報告されていることから、RMPを利活用することで、患者さんへの薬物治療のリスク回避に貢献できることが示唆された。 以上のことから、今後、RMPの利活用を広めるためにはまずその重要性を病棟担当者に啓蒙することが必要と考えられた。その後、作成したRMP記載のテンプレートを実際に使用してもらうことで、患者さんのリスク最小化につなげていきたい。 【Purpose】 The Drug Risk Management Plan (RMP) is a document that summarizes consistent risk management from drug development to post-marketing. Risk minimization activities in RMP are activities for the purpose of ensuring and improving the safety of patients who use pharmaceutical products, but the actual situation of Keio University Hospital (hereinafter referred to as our hospital) is unknown. Therefore, we aim to contribute to risk minimization by grasping the situation of our hospital and formulating measures to actively utilize RMP. 【Method】 We extracted the data of drugs for which RMP was created from the Pharmaceuticals and Medical Devices Agency (PMDA) for the drugs adopted by our hospital, and created a template to be used when giving medication guidance. In addition, we surveyed the ward staff of our hospital's pharmacy department about their awareness of RMP and the actual state of utilization of RMP. 【Results / Discussion】 For the drugs adopted at our hospital from February 2019 to April 2020, we created templates for 151 items that included the three safety considerations of RMP. At the same time, we conducted a fact-finding survey on RMP for 55 people in charge of the ward staff of our hospital's pharmacy department. The response rate was 69.1% (38/55). When asked if they were conscious of RMP in ward activities, it was found that while there were 15 people (39.5%) who were conscious, there were 23 people (60.5%) who were less conscious of the ward staff, which was more than half. It was also found that 0 people who use it often and 5 people who use it are low, while 28 people who do not use it much and 5 people who do not use it at all, which they do not usually use much. Two people using RMP reported that it could be used to treat patients, suggesting that the use of RMP can contribute to avoiding the risk of drug treatment for patients. From the above, it was considered necessary to first educate the ward staff about the importance of using RMP in order to spread its use. After that, we would like to minimize the risk of patients by having them actually use the created template described in RMP.
