• Note (General): type:text; 聴覚刺激により生じる事象関連電位は、聴性脳幹反応（ABR）より上位の聴覚中枢を他覚的に評価することができる。代表的な事象関連電位には、MMNやP300があげられる。MMNおよびP300は聴覚中枢での反応であり、聴覚刺激に対する前注意・注意と関連する聴覚情報処理過程を他覚的に評価することができると考えられ、今後の聴覚情報処理障害患者や聴覚刺激に注意障害を有する患者の評価への応用などが期待されているが、ABRと比較して検査手技が煩雑であり検査時間も長くなる。この煩雑性と検査時間の長さが、一般的な聴力検査室への普及への障壁であった。しかし、近年、検査機器の進歩により検査方法の煩雑さは軽減されてきている。 2020年度、我々は、学事振興資金研究費の補助をうけて、MMNおよびP300の測定の実臨床でのルーチン検査化を目標として、聴力正常な成人者を対象にMMNとP300検査を行い検査方法の工夫をおこなった。検査室において、この二つの検査を連続して同時に行い短時間で安定した評価が可能であるかを検討した。具体的な内容としては、MMNとP300の関電極を共通なCzに設置し時間短縮とデータの信頼性の検討を行い、信頼性のある波形が得られたが、これまでCz閾値がないため事前にCz関電極の施設内で正常者閾値を設定する必要があると考える。加算回数は、MMN、P300共200回測定で良好な波形の確認が可能であった。MMNに年代間の有意差はなく男女差、左右差も有意差は認めず安定した測定が可能であった。P300では、年代間の潜時において年齢の影響を認めたが、これは加齢により延長するという報告に一致しており今回の手法によってP300の測定において信頼できる測定が可能であったと考えられる。検査時間は、連続検査で実施しても24分で完了した。この二つの検査は、安定した評価と患者負担のない検査時間での完了が可能であった。 この研究結果は今年度、日本聴覚医学会で主題として報告した（筆頭演者：堀明美）。さらに本内容をまとめた論文を "Audiology Japan" 誌に投稿中である（2021年3月現在 in revision）。 Event-related potentials generated by auditory stimuli can objectively assess the auditory perception more up-stream than the auditory brainstem response. Typical auditory event-related potentials include mismatch negativity and P300. Mismatch negativity and P300 are reactions in the central auditory pathway, and it is considered that the auditory information processing process related to " pre-attentive attention" and "attention to auditory stimuli" can be objectively evaluated. It is expected to be applied to the evaluation of patients with attention or processing disorders. However, due to the test characteristics, which measure the long latent evoked potential, the test procedure is complicated and the test time is longer than that of ABR. The complexity of this test and the length of the test time were barriers to its widespread use in general clinical settings. In recent years, advances in equipment have reduced the complexity of inspection methods. In this report, we conducted mismatch negativity and P300 test in adults with normal hearing and examined the test method, intending to measure mismatch negativity and P300 a routine test in clinical practice. In 2020, we modified and examined for using these two methods under clinical condition easily. We conducted these two tests simultaneously in succession and examined whether the stable evaluation was possible in a short time. Finally, we reported our new methods in a domestic conference "Audiology Japan" and we submitted our results to a journal (under revision).
