• Note (General): type:text; 去勢抵抗性前立腺癌に対する治療薬として近年新たにアンドロゲン受容体シグナル阻害剤(Androgen Receptor Pathway inhibitor; ARPI)が登場したが、患者の予後が延長しているという恩恵と相反する臨床上の課題が浮き彫りとなった。それは治療誘導型神経内分泌がん（treatment-emergent Neuroendocrine Prostate Cancer: tNEPC）の出現である。tNEPCは前立腺がんの最大のアンメットメディカルニーズとして臨床上問題になってきている。本研究では、NEPC患者の血液循環腫瘍細胞(CTC)のシングルセル解析と、組織切片 における空間的遺伝子発現解析による活性化遺伝子の 3次元地図情報を統合解析することで、NEPCの新規治療戦略の確立を目的とした。本研究ではNEPC患者の進行度や時間軸に沿ってCTCを回収し、CTCシングルセル解析を実施した。シングルセルRNA, CNV解析はそれぞれ単一細胞に含まれる微量mRNAやDNAから、定量的なRNAシークエンスとCNV解析を可能にした画期的な研究手法である。 薬剤に対して感受性を有する患者群と、初回耐性を有する患者群のCTCのなかに明らかに初回耐性に関連すると分別できるクラスターを見出した。最新の空間的遺伝子発現解析システムを用いてITHを解析するための予備研究として、最適な酵素活性の時間を検討した。組織の保存状態や採取組織により、酵素活性に差を認めた。引き続き今後症例ごとの空間的遺伝子発現解析システムの最適な固定条件と解析条件の検討を進める。 Androgen Receptor Pathway inhibitor (ARPI) has recently emerged as a treatment for cast-resistant prostate cancer, highlighting clinical challenges that conflict with the benefits of prolonged patient prognosis. The purpose was to establish a strategy. In this study, we collected CTCs along the progression and timeline of NEPC patients and performed a CTC single-cell analysis. Single-cell RNA and CNV analysis is an epoch-making research method that enables quantitative RNA sequencing and CNV analysis from trace mRNAs and DNA contained in a single cell, respectively. We found clusters in the CTCs of the drug-sensitive patient group and the initial resistance patient group that could be clearly distinguished as related to the initial resistance. Using the latest spatial gene expression analysis system, we examined the optimal time of enzyme activity as a preliminary study to analyze ITH. There was a difference in enzyme activity depending on the storage condition of the tissue and the collected tissue. We will continue to study the optimal fixation conditions and analysis conditions for the spatial gene expression analysis system for each case.
