高脂血症の治療薬として用いられているHMG-CoA還元酵素阻害薬(スタチン)は抗炎症効果などの多面的な役割を有していることが報告されているが、気管支喘息に対する有用性は十分明らかになっていない。我々は気管支喘息に対するスタチンの有効性を明らかにするため、システマティックレビュー及びメタ解析を行った。1350論文のスクリーニングを行い、基準を満たした19論文(RCT 11本、観察研究 8本)を解析し、スタチンによる治療が喘息のコントロールと増悪を抑制することを明らかにし、Journal of Asthmaに報告した。本研究はスタチンが喘息の新規治療薬となる可能性を示したはじめてのメタ解析であり、今後の喘息診療の大きな転換点となる可能性があると考えられた。また、我々は活性化ILC2と非活性化ILC2のRNA-seq解析により、非活性化ILC2に特異的な遺伝子群を同定した。特に、Pdcd4遺伝子は非活性化ILC2において発現が高く、活性化に伴い発現が減少することが確認された。我々はさらにウイルスベクターを用いてPdcd4遺伝子の発現を低下させると、ILC2が活性化し、IL-5発現が上昇することを発見した。今後は詳細な機序を明らかにし、生体内での役割についても検討したいと考えており、次年度の課題と考えられた。 HMG-CoA reductase inhibitors, statins, are widely used for the treatment of hyperlipidemia. Recently, statins have been reported to have a multifaceted role in various diseases, via their anti-inflammatory effects. However, their usefulness in bronchial asthma has not been fully elucidated. We conducted a systematic review and meta-analysis to clarify the efficacy of statins in bronchial asthma. 1350 articles were screened, and 19 articles (11 RCTs and 8 observational studies) that met the inclusion criteria were analyzed. We have found that statin treatment improved asthma control and reduced exacerbations, and we reported these findings in the Journal of Asthma. We also identified a group of genes specific for non-activated ILC2s by RNA-seq analysis. In particular, the Pdcd4 gene was found to be highly expressed in non-activated ILC2, and its expression decreased upon activation. We also found that down-regulation of Pdcd4 gene expression using viral vectors resulted in activation of ILC2 and up-regulation of IL-5 expression. We are going to elucidate the detailed mechanism and study the role of Pdcd4 in ILC2s in vivo next year.
