• Note (General): type:text; 【目的】慶應義塾大学病院（以下、当院）において、医薬品の処方オーダー時の食物アレルギー物質の注意喚起は不十分であり、インシデントも発生している。また、添付文書に記載されていなくても、添加物等の原料に食物アレルギー物質が含まれていることもある。本研究では、医薬品の食物アレルギー表示の実態調査を行うことで、処方オーダー時の院内チェックシステム構築に向けての問題点を抽出する。 【方法】当院取り扱い医薬品について、製造販売元に対し、食品衛生法及び消費者庁次長通知の「特定原材料」、「特定原材料に準ずるもの」に該当する品目の含有の開示を依頼し、得られた報告を分析した。 【結果】情報開示を受けた医薬品は1,453薬剤であり、その内訳は、食物アレルギー物質含有が668剤（46.0％）、無746剤（51.3％）、不明3剤（0.2％）、無回答36剤（2.5％）であった。原因物質として最も多かったのは乳であり、情報開示を受けた医薬品1,453品目中、461品目（31.7％）を占め、次いでゼラチン、大豆が多かった。 【考察】食物アレルギー物質が含有されていても、安全性上の最大許容値以下での使用や精製により除去されていると判断して、添付文書には記載していない製薬会社がある一方で、添加物の由来原料が追跡できないため、食物アレルギー物質の含有を否定できないと回答した製薬会社もあった。添付文書に記載がなくても、由来原料に食物アレルギー物質を含有している医薬品が多く存在する可能性が考えられた。食物アレルギー物質含有医薬品を全て正確に把握出来なかった要因の一つとして、医薬品の食物アレルギー表示の統一した見解が定められておらず、製薬会社により解釈が異なることが考えられた。 【結論】食物アレルギー患者により安全な薬物療法を提供するために、医薬品の食物アレルギー表示の法整備が必要と思われた。当院では、食物アレルギー物質含有を把握できた医薬品は処方オーダー時に注意喚起を促す表示を行う予定である。 [Purpose] At Keio University Hospital (hereinafter referred to as our hospital), attention to food allergens is insufficient when ordering medications, which results in allergic reaction incidents. In addition, food allergens may be contained in raw materials such as additives, even if they are not described in the package insert. In this study, we conduct a survey on food allergy labeling of pharmaceutical products in order to extract issues for configuring in-hospital triage support system for safe medication ordering. [Methods] We requested the manufacturers and distributors to disclose the contents of items containing in pharmaceutical products on our hospital formulary, that fall under the "Specified Raw Materials" or "Similar to Specified Raw Materials," notified by the Food Sanitation Law and the Deputy Director of the Consumer Affairs Agency. [Results] The number of drugs for which information was disclosed was 1,453, of which 668 (46.0%) of the drugs contain food allergens, 746 (51.3%) were non-inclusion, three (0.2%) were unknown, and 36 (2.5%) were disclosed as "no answers." Milk was the most common raw material, accounting for 461 (31.7%) out of 1,453 drugs, followed by gelatin and soybeans. [Discussion] Even if food allergens are contained, some pharmaceutical companies do not list them in the package insert because they judge that food allergens have been removed by use or purification to a level below the maximum tolerance allowable. other pharmaceutical companies answered that they could not deny the inclusion of food allergens because the raw materials derived from the additives could not be tracked. Even if any food allergens are not listed in the package insert, some pharmaceutical products may contain food allergens in their derived raw materials. One of the possible factors that made it impossible to accurately grasp all food allergen-containing drugs is that a unified view of food allergy labeling of drugs has not been established, and that the interpretation differs depending on the pharmaceutical company. [Conclusion] In order to provide safer pharmacotherapy to patients with a history of food allergies, it is necessary to establish legal regulations for labeling food allergies. At our hospital, an alert will be displayed on the electronic chart to call attention when ordering medications containing food allergens for patients with a history of the food allergies.
