• Note (General): type:text; 研究の目的 ポリファーマシーは多剤併用の中で害をなすものと定義され、転倒や薬物有害事象の増加に繋がるため解決が望まれている。2020 年度の診療報酬改定で薬剤総合評価調整加算が見直され、ポリファーマシーに対して多職種と連携し処方内容の変更を行った場合に100 点、2種類以上減薬された場合はさらに150点が算定出来るようになったが、当院では当該加算の実績がないのが現状である。本研究では薬剤総合評価調整加算の算定体制を構築することを目的とする。 研究の内容 当院薬剤師のポリファーマシーおよび薬剤総合評価調整加算に対する意識・理解度についてアンケート調査（実施期間：2020年9月18日～9月29日）を実施し現状および問題点を把握した。その後ポリファーマシーの基礎的知識や診療報酬についての勉強会、患者への介入方法などの勉強会を2回開催し、算定要件の周知、意識づけを行った。勉強会実施後に2回目のアンケート調査（実施期間：2021年3月5日～3月19日）を実施し、取り組みの成果を調査した。また、薬剤総合評価調整加算の算定件数を調査した。 研究の成果 2回のアンケートでそれぞれ48名の薬剤師の回答を得た。ポリファーマシーの検出、対応方法の代表的な資料である「高齢者の安全な薬物療法ガイドライン2015」について内容を概ね把握している割合は1回目では39％に留まったが、2回目では60%まで上昇した。また、薬剤総合評価調整加算の算定要件についての理解度は、普段から病棟業務に携わる薬剤師（1回目37名、2回目43名）で33%から63%へと上昇し、ポリファーマシーや加算への理解が広まっていると推察された。一方、加算を算定出来ない理由として、入院期間が短く介入が難しいと約70%が回答し、当院のような急性期病院では、短い入院期間での介入方法の整備が今後の検討課題として考えられた。また、本年度の薬剤総合評価調整加算は44件、薬剤調整加算（150点）は4件の算定であった。 1.Background Polypharmacy is a factor affecting the increase in adverse medicine reactions and falls. With the revision of medical fees in 2020, the comprehensive medicine evaluation adjustment addition was revised. If the prescription with polypharmacy was changed in collaboration with multiple occupations ,100 points were given, and 150 points were added when two or more medicines were reduced. However, in Keio University Hospital, the comprehensive medicine evaluation adjustment addition was no record. In this research, I developed a method to the comprehensive medicine evaluation adjustment addition. 2.Method I conducted a questionnaire survey (implementation period: September 18-29, 2020) for pharmacist. This purpose is to know the degree of recognition and understanding about polypharmacy and comprehensive medicine evaluation adjustment addition in our hospital. Refer to the result, I held two study sessions on basic knowledge of polypharmacy, medical fees, and intervention methods for polypharmacy patients. After the study session, the second questionnaire survey (implementation period: March 5, 2021 to March 19, 2021) was conducted to investigate the results of the efforts. In addition, we investigated the changes in the number of the comprehensive medicine evaluation adjustment addition. 3.Result The degree of general understanding of the "Guidelines for Safe Pharmacotherapy for the Elderly 2015," which is a representative guideline for screening and intervention against polypharmacy, increased from 39% to 60% (n=48). In addition, the degree of understanding of the calculation requirements for the comprehensive medicine evaluation adjustment addition increased from 33% to 63% for pharmacists who are usually involved in ward work (n=37 for the first time and n=43 for the second time). It was speculated that the understanding of polypharmacy and addition was widespread. On the other hand, about 70% pharmacists answered that the hospitalization period is short and it makes difficult to intervene as the reason why the calculation cannot be taken. I will prepare flow chart for intervention. In addition, 44 cases of comprehensive evaluation adjustment addition (100 points) and 4 cases of total medicine adjustment addition (150 points) were calculated this year.
