• Note (General): type:text; 本研究の目的は、がん薬物療法および化学療法誘発性末梢神経障害（chemotherapy-induced peripheral neuropathy:以下 CIPN)を有する患者の転倒の実態や転倒予防に関するケアについてシステマティックレビューを行い、看護師への教育プログラムの内容を検討することであった。データベースはPubMed，医中誌Web版を用いた。検索期間は2016年から2021年の期間とし、「がん」「化学療法誘発性末梢神経障害/CIPN」「転倒」をキーワードとした。その結果、PubMedより39件、医中誌より19件が得られ、タイトルと抄録からスクリーニングを行い、ハンドサーチより最終的に8件を抽出した。研究デザインは、横断研究が7件、コホート研究が1件であった。研究対象者は、タキサン系の治療を受けたがん患者を対象とした研究が2件、タキサンまたはプラチナ系の治療を受けた患者を対象とした研究が1件、CIPNを有する患者を含めがん患者を対象とした研究が5件であった。CIPNと転倒の関連要因を調査した研究が5件、CIPNを有する患者の歩行状態を明らかにした研究が2件であった。CIPNを有する患者の転倒の実態として、症状がない患者と比較し、1.8倍から3倍程、転倒やけがを引き起こしやすく、段差のない平らな場所や明るい場所、屋内での転倒が多いことが示唆された。また、CIPNを有する患者の歩行状態について、小さな歩幅でゆっくりとした歩行速度が特徴として挙げられた。また、手や上肢の機能障害や末梢神経障害の強度、運動神経障害やロンベルグ徴候を認めることが明らかになった。以上より、CIPNを有する患者の歩行状態や転倒の実態を踏まえたアセスメントや患者教育の必要性が示唆された。教育プログラムの具体的内容や方法の検討まで至らなかったため、今後の課題である。 The purpose of this study was to conduct a systematic review of fall prevention care for patient with chemotherapy induced peripheral neuropathy (CIPN). The literature search was conducted using the search databases "Ichushi Web, the Japanese medical published work database" and "PubMed", with the keywords "cancer", "chemotherapy-induced peripheral neuropathy/CIPN", and "falls". The publication year was from 2016 to 2021. As a result, 39 articles were obtained from PubMed and 19 from medical journals, screened by title and abstract, and finally 8 articles were extracted from the hand search. The study design included seven cross-sectional studies and one cohort study. Participants included two studies of cancer patients treated with taxanes, one study of patients treated with taxanes or platinum, and five studies. Five studies investigated factors associated with CIPN and falls, and two studies clarified the gait status. The results suggest that patients with CIPN are 1.8 to 3 times more likely to fall or injure than patients without symptoms, and that there were more falls in flat places, bright places, and indoors. The gait condition of patients with CIPN, a slow gait speed with small strides was noted as a characteristic. In addition, it was revealed that dysfunction of hands and upper limbs, intensity of peripheral neuropathy, motor neuropathy, and Romberg's sign. These findings suggested that assessment and patient education based on the gait status and falls of patients with CIPN are necessary. The future task is to examine the specific contents and methods of the educational program.
