• Note (General): type:text; 新型コロナウイルス（以下、新型コロナ）の感染が世界的に拡大してから１年経過し、その間に、わが国の医療提供体制の脆弱性が明らかとなった。体制が脆弱であったため、新規感染者及び重症者の増加を抑える必要から、経済活動を抑制せざるを得なくなった。そして、経済活動を抑制することへの補償的な措置として、給付金等を用いた財政政策が講じられた。 ３年計画の２年目における研究として、経済活動の抑制の起因となったわが国の医療提供体制の脆弱性について、マクロ経済・財政面から考察を行った。そこでは、入院医療において、機能分化が十分に進んでいないことから低密度医療となっており、それが新型コロナ対応においても、コロナ病床の確保に苦労する一因となった。そして、そうした病床配置は、出来高払いの診療報酬体系、さらには包括払いの診療報酬があれども、入院１日当たりの包括払い（PDPS）となっていることが、それを助長していたことが示唆される考察結果を得た。 加えて、今年度は、日本家計パネル調査（JHPS）のデータを用いた分析準備を行った。これは、新型コロナ感染拡大期に、わが国で給付金等による家計への支援が実施され、その経済効果を分析するためである。ただ、予定していたデータクリーニング・分析の補助が、人員確保ができなかったために行えず次年度に実施することとした。 After the global spread of the COVID-19, vulnerability of healthcare delivery system became apparent in Japan. As a result, the need to control the increase in the number of newly infected and patients with severe COVID-19 has forced us to suppress economic activities. As a compensatory measure for this suppression of economic activities, fiscal policy was implemented through the provision of benefits and other means. In the second year of the three-year plan, I examined the vulnerability of Japan's healthcare delivery system, which was the cause of the curtailment of economic activity, from viewpoints of macroeconomy and public finance. The study found that inpatient care was low-density due to insufficient functional differentiation, which caused to the difficulty in securing beds for the COVID-19. The results of the study suggest that the allocation of such beds was promoted by the fact that the reimbursement system for medical services was based on fee-for-service, and even if there was a comprehensive reimbursement system, it was based on a per-diem payment system (PDPS). In addition, this year I prepared an analysis using data from the Japan Household Panel Survey (JHPS). This was done in order to analyze the economic effects of the support provided to households in Japan through benefits and other means during the period of the spread of the COVID-19. However, data cleaning and analysis assistance could not be performed due to lack of staffing as planned and will be performed in the next year.
