• Note (General): type:text; 潰瘍性大腸炎患者から樹立したオルガノイドからDNAを採取し、これらの全エキソンシークエンスを行ったところ、潰瘍性大腸炎においてIL-17シグナル遺伝子に集積して遺伝子変異が生じていることが分かった。このうちPIGRは、二量体IgAを基底膜側より管腔側に移送する分子であり、これらを上皮特異的にノックアウトしたマウスでは実験的腸炎の病勢が悪化することが報告されている。私達は、この遺伝子が炎症部に特に多く発生し、またその変異頻度が担癌症例において高いことを見出している。これまでに、内視鏡での経時的な炎症度を積分的に加算しその総和が発がんのリスクとなる報告も見られている。そこで、解析したサンプルの遺伝子変異の数と内視鏡での炎症度を比較したところ、内視鏡で比較的炎症が落ち着いていた症例においても、遺伝子変異が多く生じているサンプルがあり、それらの症例においてはPIGRなどのIL-17シグナル遺伝子の変異が生じていることがわかり、これらの遺伝子変異は内視鏡による定点的な観察よりも、よりこれまでの炎症を反映している可能性が考えられた。 そこで、さらなる検証のため、これまでに当院で診断された腸炎関連がん患者のうち診断の1-3年ほど前の内視鏡検査および生検による病理検査がされていた症例を選定し、これらの病理サンプルを用いてPIGRの免疫化学染色を行い、その変異頻度を見てみたところ、約半数の症例ではPIGRの陰性領域が確認された。 今後、同時期に行われた非発がん症例の検討をすすめ、この分子の変異が発がんリスクバイオマーカーとして有用であるかを検証する。 We extracted DNA from organoids established from patients with ulcerative colitis and subjected them to whole exon sequencing. We found that genetic mutations accumulated in IL-17 signaling genes in ulcerative colitis. It has been reported that epithelial-specific knockout of PIGR, a molecule that transports dimeric IgA from the basement membrane to the luminal side, worsens the disease course of experimental colitis in mice. We have found that this gene is particularly prevalent in the inflammatory regions and that its mutation is more frequently found in cases with colitis associated carcinoma. It has been reported that the risk of carcinogenesis is calculated as the sum of the cumulative total of the inflammation levels over time in the endoscope. We compared the number of gene mutations in the analyzed samples with the degree of endoscopic inflammation, and found that even in cases where the inflammation was relatively stable during endoscopy, there were some samples with a large number of gene mutations, and in these cases, mutations in IL-17 signaling genes such as PIGR occurred. Therefore, it is thought that these IL-17 signaling gene mutations may reflect the inflammation more than the endoscopic findings. To verify this , we performed immunochemical staining for PIGR in pathology samples from patients with colitis associated carcinoma who had been diagnosed at our hospital and had undergone pathological examination by endoscopy about 1-3 years prior to diagnosis of carcinoma. About half of the cases showed a negative PIGR region. In the future, we will continue to examine non-cancerous cases performed in the same period to verify whether mutations in this molecule are useful as a carcinogenic risk biomarker.
