• Note (General): type:text; (目的) PD-1抗体により、がんと闘うTリンパ球（T細胞）を活性化させ、多くの患者を救える事が示された。現在のところ、PD-1抗体単剤の奏効率は20－30％にとどまり、この改善が求められている。抗PD-1療法は、T細胞が「異物」として認識するがん抗原が多いほど高い奏効率を示すが、がん細胞にこのような新規抗原を誘導する薬剤は無い。私は、体細胞でクロマチン抑制因子であるTRIM28を欠損させると、通常はサイレンシングを受けている、様々な遺伝子が発現することを見出した。本研究では、以下の実験を行い、TRIM28の機能的阻害剤を開発し、がん細胞に新規抗原誘導することを目標として研究を行っている。 (結果と考察) TRIM28リン酸化酵素阻害剤の、がん免疫における効果 TRIM28分子内にある、セリン４７３残基は、脱リン酸化、リン酸化によってTRIM28による遺伝子抑制をon/offに制御する「スイッチ」の役割を果たすことが知られている。昨年までに、このリン酸化修飾を起こすキナーゼとしてProtein Kinase D （PKD）を同定した。がん移植マウスにＰＫＤ阻害薬を投与し、ＰＤ－１抗体との併用による腫瘍の抑制効果を検討した。ＰＤ－１抗体が腫瘍を顕著に抑制したのに対し、予想に反してPKD阻害剤投与群では腫瘍成長がむしろ促進し、ＰＤ－１治療の効果もキャンセルしてしまう事がわかった。原因を追究した所、PKD阻害薬はTRIM28のリン酸化に加え、ＰＤ－１治療でＴ細胞増強に必須と考えられるＡＫＴキナーゼの活性をも弱めてしまう事がわかった。一方、PKD阻害薬が単回投与でT細胞免疫を強力に抑制するという結果が得られ、過剰なT細胞活性化が起こす、自己免疫疾患などに対して治療効果を示す可能性が示された。 (PURPOSE) Immune checkpoint therapy by anti-PD-1 antibody was shown to activate T lymphocytes (T cells) that fight against many types of tumor and improve survival of cancer patients. To date, the overall response rate of anti PD-1 monotherapy is limited to ~30%, which improvement is desired. It is known that the amount of tumor neoantigen correlate with efficacy of anti-PD-1. We found that TRIM28 deficiency, a global chromatin silencing factor, resulted in expression of genes that are usually silenced in mouse somatic cells. The purpose of this study was to find TRIM28 inhibitors by screening, and test them if they improve the efficacy of immune checkpoint therapy. (RESULTS AND DISCUSSION) By kinase inhibitor screen, we identified a protein kinase D (PKD) that phosphorylates S473 residue of TRIM28. We carried out mouse tumor transplantation model to test whether PKD inhibitor augments antitumor response together with anti-PD-1. Unexpectedly mice treated with PKD inhibitor did not inhibit, but rather accelerated tumor growth, and reversed the therapeutic effect of anti-PD-1. Mechanistically, PKDi inhibited phosphorylation of AKT, a primary checkpoint that is reactivated by anti-PD-1. In conclusion, PKD is fundamentally required for T-cell reactivation by anti-PD-1, therefore inhibition of PKD is not appropriate for combination therapy with anti-PD-1. However a single dose of PKDi was shown to strongly suppress ongoing T-cell reaction, indicating that PKDi could be useful for treatment of immune related adverse events, that are frequently reported in anti-PD-1 therapy.
