• Note (General): type:text; 研究を開始するに当たり、川崎病を数多く診療している、茨城県の筑波メディカルセンター病院、大阪大学病院小児科およびその関連施設を共同研究機関として倫理申請を行い、承認を得た。共同研究機関を通じて、川崎病患者25名から、咽頭ぬぐい液を採取し、専用容器に保存し、ウイルスを失活させ収集することができた。 効率的なウイルス、細菌、真菌由来のDNA断片を収集するため、健常者の咽頭ぬぐい液を用いたDNA収集を試みた。製造者が指定するプロトコールで実施したところ、DNA収量が感度以下であり、工夫を要した。インキュベートの時間、細胞破砕を行う時間と回転数、温度の調整を行い、メタゲノム解析可能な量のDNA収集に成功した。 収集したDNAのライブラリ調整を行い、自研究室でのショットガンシーケンスを予定していたが、ショットガンシーケンスは１回の解析で、約50検体分のシーケンスが可能であり、検体数が不足していたため、実施は延期することとし、症例の集積を待つこととした。 シーケンス後の解析手法について、既知の病原体を同定することは確立した。ショットガンメタゲノム解析は、検体より抽出されたDNA/RNAは全ゲノムを断片化し、PCR増幅を行い、次世代シーケンサーを用いてリード（塩基配列）を取得する。得られたリードの共通する配列をつなぎ合わせそれぞれのリードがどの微生物に由来するのかを参照ゲノムと比較することで細菌、ウイルス、真菌に詳細に分類する。これまでは、脳炎・髄膜炎患者の髄液検体や、肺炎患者の喀痰検体といった感染巣そのものから抽出したDNAによる解析であったが、ヘルペスウイルス肝炎患者の血液検体からでもウイルスゲノムを特定することに成功した。量の少ないDNA量からでもウイルスの同定が可能なパイプラインを整えた。 今後、検体の集積をまって、ショットガンメタゲノムシーケンスを実施し、結果を導出する予定である。 To begin the study, we submitted an ethics application to Tsukuba Medical Center Hospital in Ibaraki Prefecture, the Department of Pediatrics of Osaka University Hospital, and their affiliated facilities, which treat a large number of patients with Kawasaki disease, as joint research institutions, and obtained their approval. Through these institutions, pharyngeal swab fluid was collected from 25 Kawasaki disease patients and stored in special containers to inactivate the virus for collection. In order to efficiently collect DNA fragments derived from viral, bacterial, and fungal sources, we attempted to collect DNA using pharyngeal swab fluid from healthy subjects. When the protocol specified by the manufacturer was used, the DNA yield was less than sensitive, and ingenuity was required. After adjusting the time of incubation, the time and number of rotations for cell disruption, and the temperature, we succeeded in collecting a metagenomic-analysis-ready amount of DNA. We adjusted the library of collected DNA and planned to perform shotgun sequencing in our laboratory, but since shotgun sequencing can sequence about 50 samples in one run and the number of samples was insufficient, we decided to postpone the implementation and wait for the accumulation of cases. Regarding post-sequencing analysis methods, identification of known pathogens was established. In shotgun metagenomic analysis, DNA/RNA extracted from specimens is fragmented from the entire genome, PCR amplification is performed, and reads (nucleotide sequences) are obtained using a next-generation sequencer. The common sequences of the obtained reads are stitched together and compared to a reference genome to determine which microorganism each read is derived from, and the reads are then classified into bacteria, viruses, and fungi in detail. Until now, analysis has been based on DNA extracted from the infection sites themselves, such as spinal fluid samples from patients with encephalitis or meningitis, or sputum samples from patients with pneumonia, but now the virus genome has been successfully identified even from blood samples from patients with herpesvirus hepatitis. The pipeline has been established to enable identification of the virus even from small amounts of DNA. We plan to conduct shotgun metagenomic sequencing after accumulating more samples and derive the results.
