• Note (General): type:text; 当院倫理審査で承認済みの研究課題（承認番号：20200057）を施行するための独自MREシステムを完成させた。現在、臨床で使用されているMREシステムは肝臓に特化しているため、任意の組織にMREを適用することは困難である。一方、自作した本MREシステムでは3Dプリンタを用いて対象組織に応じて適切な形状の振動パッドを作成することで、肝臓以外の対象においてもMREの臨床応用を進めることが可能である。今年度で得られた成果の一部は、来年度の第78回日本放射線技術学会総会学術大会と第95回日本整形外科学会学術総会にて発表予定である（演題採択済み）。 第78回日本放射線技術学会総会学術大会においては、開発した弾性率自動解析方法CHASE（Daiki Ito et al. Magn. Reson. Imaging, 2022.）の面内空間分解能による依存性の検証を肝臓及びファントムを対象に行なったものである。面内空間分解能の異なる画像に対して、MREを熟知している観察者による手動計測とCHASEによる自動計測の一致度を比較したところ、両者に有意な差はみられなかった。本研究により、MRE画像の面内空間分解能が変化した場合においても、CHASEは高精度な弾性率測定が可能であることを実証した． 第95回日本整形外科学会学術総会においては、棘上筋に対してMREを適応できるシステム開発を行った成果を報告する。腱板断裂は通常、棘上筋腱の損傷から始まる。その主な要因は、オーバーユースによる筋収縮であり、負荷の初期段階では筋硬度が上昇する。しかし、現臨床において棘上筋の硬度を定量的に評価できる技術は確立されていないため、新たにMREの棘上筋適応を試みた。3Dプリンタによる自作振動パッドを肩の曲率に合わせることで、被験者に依存せずに棘上筋MREを実行できた。 今後、新しいMRE撮像方法に関する特許出願も予定しており、上記のようなMREの臨床応用と基礎研究を並行して進めることで、MREに関する新たな知見を明らかにしていく。 We developed an original MRE system to conduct a research project (approval number: 20200057) that has already been approved by institutional ethics review board. Clinically, it is difficult to apply MRE to various tissues because clinical MRE system is specialized for the liver. On the other hand, our MRE system can be applied to various tissues by using a 3D printer to make a vibration pad with an appropriate shape according to the target tissue, thus making it possible to promote the clinical application of MRE. The research finding in this year are scheduled to be presented at the 78th Annual Meeting of the Japanese Society of Radiological Technology and the 95th Annual Meeting of the Japanese Orthopaedic Association (abstracts have already been accepted). In the 78th Annual Meeting of the Japanese Society of Radiological Technology, we report the dependence of CHASE, an automated shear modulus measurement system (Daiki Ito et al. Magn. Reson. Imaging, 2022.), on the in-plane spatial resolution of liver and phantom. In this study, we compared the agreement between manual measurements by expert observers and automatic measurements by CHASE for MRE images with different in-plane spatial resolutions, and found no significant difference between them. This study demonstrates that CHASE is independent of the in-plane spatial resolution of MRE images. In the 95th Annual Meeting of the Japanese Orthopaedic Association, we report the results of our work to develop a system that can adapt MRE for the supraspinatus muscle. Rotator cuff tears usually begin with an injury to the supraspinatus tendon. The main factor is muscle contraction due to overuse, and muscle stiffness increases during the initial phase of loading. However, since the technique to quantitatively evaluate the stiffness of the supraspinatus muscle has not been established in current clinical practice, we attempted a new MRE technique of the supraspinatus muscle using our MRE system. The results of this study were indicated that the MRE of the supraspinatus muscle can be performed without depending on the subject. In the near future, we plan to file a patent application for a new MRE imaging method. Moreover, we will acquire new knowledge about MREs by promoting the clinical application of MREs as described above and basic research in parallel.
