• Note (General): type:text; がん患者の多くは化学療法などの副作用として便秘を生じる。近年、その対策に使用される酸化マグネシウム製剤服用により死亡に至るほどの高マグネシウム血症症例が報告されたことから、その適正使用には注意を要する。 我々は今までに、5-FU などの抗がん剤誘発性消化管障害時に併用経口薬物の消化管吸収が変動することを明らかにしており、抗がん剤施行時には酸化マグネシウム製剤の消化管吸収も変動する可能性がある。 そこで本研究では、ラットを用いて、抗がん剤投与時に酸化マグネシウム製剤併用投与した際の体内動態を評価することを目的とした。 まず、5-FU 30 mg/kg/day を 5 日間反復経口投与することで 5-FU 誘発性消化管障害モデルラットを作出した。5-FU 投与開始から 6 日目に酸化マグネシウム製剤として 80 mg/kg を経口投与し、24 時間後まで経時的に採血および蓄尿を行った。血漿中および尿中のマグネシウム濃度を定量し、各薬物動態パラメータを算出した。その結果、5-FU 誘発性消化管障害モデルラットではコントロールラットと比較して、腎クリアランスが 65% にまで低下したにもかかわらず、血漿中濃度-時間曲線下面積は同等であった。よって、5-FU 投与により、マグネシウムの吸収は低下し、マグネシウムの全身移行量は 73% にまで低下することが示された。 以上より、5-FU 投与時においては、マグネシウムの消化管吸収が低下することが示唆された。今後は 5-FU 以外の抗がん剤についても検討していくことで、本知見が一般化可能か検証していく予定である。また、抗がん剤投与時の抗菌薬の体内変動についても引き続き検討していく。 Many cancer patients experience constipation as a side effect of chemotherapy. Recently, a case of hypermagnesemia resulting in death was reported after taking magnesium oxide, which is used as a countermeasure for constipation, so caution is required in its proper use. We have previously shown that gastrointestinal absorption of concomitant oral drugs is altered during anticancer drug-induced gastrointestinal damage such as 5-FU, and gastrointestinal absorption of magnesium oxide preparations may also be altered during anticancer drug administration. This study aimed to evaluate the pharmacokinetics of magnesium oxide preparations administered in combination with anticancer drugs in rats. The pharmacokinetic parameters were calculated in a rat model of 5-FU-induced gastrointestinal tract injury after oral administration of 80 mg/kg of a magnesium oxide preparation. The results showed that the area under the plasma concentration-time curve was similar in the 5-FU-induced gastrointestinal injury model rats compared to the control rats, even though the renal clearance was reduced to 65%. Thus, 5-FU administration decreased the absorption of magnesium, and the systemic transfer of magnesium was reduced to 73%. These results suggest that gastrointestinal absorption of magnesium is decreased when 5-FU is administered.
