• Note (General): type:text; ラメルテオンによる薬剤性腎機能障害の予防効果を評価するために、その第一段階としてバンコマイシンによる急性腎機能障害に着目して研究を開始した。その成果を基礎研究と臨床研究に分け、以下の通り記述する。 ①基礎研究（動物実験） 研究の準備段階として、バンコマイシンによる急性腎機能障害のモデルラットの作成に着手した。まず、動物実験委員会に研究計画を提出し、承認を受けた。具体的にはラットにバンコマイシンを投与した群と、生食をコントロールとして投与した群（各6匹、計12匹）を作成し、血清クレアチニン値と尿素窒素値の変化を観察することとした。その結果、尿素窒素値についてはバンコマイシン投与群において顕著な上昇が認められた。今後はTBARSアッセイなどを用いた酸化ストレスの評価系を確立するとともに、ラメルテオンの経口投与実験を行い、その腎機能障害抑制効果を検証していく。 ②臨床研究（ビッグデータ解析） 医療ビッグデータを用いて、バンコマイシンによる急性腎機能障害をラメルテオンが抑制するかを検証した。まず、倫理委員会に研究計画を提出し、承認を受けた。次に、一般社団法人健康・医療・教育情報評価推進機構（HCE-I）が構築した診療情報データベースを取得し、バンコマイシン投与患者（n= 643,48）を抽出した。そのうち8,390例（13.0%）に腎機能障害が発現し、この割合は既報と同程度であることを確認した。今後は「年齢、体重、併用薬、基礎疾患」などの様々な共変量を調整することにより、本研究の仮説が真のものであるかを検証していく。 以上、この度の学事振興資金（個人研究）の支援により、実験動物のモデル作成ならびに解析データセットの構築の2点を行うことができた。立ち上げの段階であることから、外部発表に足る成果は出せていないものの、今年度の成果を生かし、次年度以降も研究を発展させていく所存である。 To evaluate the efficacy of ramelteon in preventing drug-induced kidney injury, we initiated a study focusing on vancomycin-induced acute kidney injury as the first step. The results are categorized into basic research and clinical research and described as follows: 1. Basic research (animal study) As a preliminary step of the research, we initiated the preparation of a rat model of vancomycin-induced acute kidney injury. First, we submitted a research protocol to the Animal Experimentation Committee and received approval. Specifically, the rats were divided into a vancomycin group and a sham group (6 rats each, 12 rats in total), and observed for changes in serum creatinine and urea nitrogen levels. As a result, a significant increase in urea nitrogen levels was observed in the vancomycin group. In the future, we will establish an evaluation system for oxidative stress using the TBARS assay, etc., and perform oral administration experiments of ramelteon to verify its effectiveness in preventing kidney injury. 2. Clinical Research (big data analysis) Using medical big data, we verified whether ramelteon prevents acute kidney injury caused by vancomycin. First, the research protocol was submitted to the Ethics Committee and approved. Next, we obtained a large Japanese electronic medical record database established by the Health, Clinic, and Education Information Evaluation Institute (HCEI; Kyoto, Japan), and extracted vancomycin injected patients (n= 643,48). Of these, 8,390 (13.0%) developed kidney injury, and we confirmed that this proportion was similar to previous reports. In the future, we will examine whether the hypothesis of this study is valid by adjusting various covariates such as "age, weight, concomitant medications, and underlying diseases". With the support of Keio University Academic Development Funds for Individual Research, we succeeded in creating an experimental animal model and constructing a data set for analysis. Although we have not yet achieved enough results to be presented externally because we are still in the start-up phase, we will continue to develop our research in the following fiscal years and beyond, based on the results of this year's research.
