臨床現場では、承認を取得して供給されている医薬品だけでは必ずしも十分ではない場合がある。院内製剤は薬事承認を取得していない製剤を調製・使用するものであり、薬物治療における院内製剤の必要性を示されている。院内製剤は製造物責任法など関連法規に準拠した対応が求められており、無菌製剤である点眼剤は薬効成分の安定性に加えて無菌性の担保も求められるがそれらの知見は限られている。本研究では院内製剤の点眼剤に関する安定性および無菌性の担保を目的として、有効成分の残存率および無菌性を調査する。院内製剤点眼剤の中でも、デノシン点眼液は長期保管により結晶が析出することが問題視されており、当院の患者からも同様の報告が挙がっているため本研究の調査対象とした。高速液体クロマトグラフィー（HPLC）による力価測定、pH、無菌性について長期的な変化を評価することとした。既報で調製後3ヶ月は安定性が保証されており、試験期間を当初の期間より延長し12ヶ月までとした。初めに、デノシン点眼剤の測定系の確立およびHPLC測定におけるバリデーションを確認した。デノシン点眼剤の有効成分であるガンシクロビルの原薬を測定したところ、保持時間が2.5分であった。デノシン点眼剤と同様の調製方法で調製したものを測定したところ、保持時間は2.5分であり、同一の保持時得られた。また、絶対検量線法でガンシクロビルの濃度測定が可能であることを確認した。今年度は当院でのデノシン点眼剤のバリデーションを最小限にする測定系を確立することができたため、今年度に確立した手法をもとに来年度に本試験を実施し、長期安定性を評価する。長期安定性が確保できれば、安定である期間に準じてデノシン点眼剤の使用期限の延長を検討する。また、析出した点眼剤の提供があれば随時測定を実施する。 In clinical practice, approved medicines alone may not be enough. In-hospital preparations are off-label use, but those are needed in clinical practice. In-hospital preparations are required to comply with relevant laws such as the Product Liability Law. Stability of the active ingredients of in-hospital prepared eye drops and sterility must be ensured, but there are few findings over 3 months. Also, Crystals of ganciclovir (GCV) which were prepared for in-hospital prepared eye drops have been reported to precipitate during storage, and patients in our hospital have reported the same problem. In this study, we investigate the long-term efficacy and sterility of in-hospital eye drops. We decided to evaluate long-term stability in GCV concentration by high-performance liquid chromatography (HPLC), pH, and sterility. The study period was set to 12 months. First, we determine the method for GCV eye drops and validation of the HPLC assay were researched. Retention time of GCV was 2.5 minutes measured by using HPLC. The retention time which is GCV eye drops was almost same time. We confirmed that it is possible to measure the concentration of ganciclovir using the absolute calibration method. We were able to determine the method of GCV eye drops stability by measuring its concentration. To confirm whether extending expiration date, we will conduct long-term study in the future.
