• Note (General): type:text; 知覚・感情・言語による包括的ケア技術（ユマニチュード）の認定インストラクターによるユマニチュード®（ケアコミュニケーション技術）実践者育成4日間研修（以下、研修とする）を6名の看護師に実施した。研修の前後で、各研修者を対象に①模擬患者を用いた職務遂行（日常業務の場面）デモンストレーションの実施をビデオ撮影、また➁JSPE Empathy score（共感度スコア） を用いて職務に関する心理学的評価を行い、次のような結果が得られた。 上記①では、「見る」（ケアする人が模擬患者の視界に写り、ケアする人も模擬患者を見ている状態）、「話す」（ケアする人が発話している状態）、「触れる」（コミュニケーション目的で模擬患者の体に触れている及びケア目的で体に触れている状態）を行っていた時間の割合がいずれも研修後に増加した。また「見る」「話す」「触れる」を同時に2つ以上使用するマルチモーダルコミュニケーションを行っている時間の割合についても、研修後に増加した。➁については、共感度スコアは全員が上昇していた。研修後は、情緒的消耗感、脱人格化について減少がみられ、脱人格化においては統計的有意差が見られた。また個人的達成感は、研修後、優位に増加していた。 その他、研修を受けて実際の臨床場面での変化を感じたかについては、マルチモーダルコミュニケーションを実践することで、患者との絆を構築することが容易になったと感じる、ケアを拒否していた患者がケアに協力的だったなど、患者の反応が変化したことを述べていた。また怒りなど感情的な反応を示す患者に対する関わり方が分かり、気持ちが楽になったなど、ケアをする側も変化を感じていることがわかった。 今後、更に評価結果を分析し、専門職を対象としたケアコミュニケーション技術教育法の効果について検証していく。 6 nurses participated 4-days-training program regarding the structured sequence of Humanitude care procedures is based on relationship pillars: gaze, speech, touch; by the Humanitude Care Methodology (HCM) certified instructor. Before and behind the training, (1) the films were shot about the participants who demonstrated daily nursing care for simulated patients, and (2) psychological evaluations were assessed with JSPE Empathy score. The result was as follows. In a case of (1), the percentage of time that the trainees have one of gaze, speech or touch was increased after the program. Additionally, the percentage of time that the trainees approached with using two or more the relationship pillars, which are called multimodal comprehensive care, was also increased. For 2), JSPE Empathy score improved after the program. Moreover, the decrease in emotional exhaustion and depersonalization was shown to the participants, and it showed a statistically significant effect on the depersonalization. Also, Personal accomplishment was increased preferentially at the post-training. Other than the quantitative results, the participants obtained results for change of sense in the real clinical situation after the program as qualitative evaluation, such as sense of ease to build the bond with a patient, and obtainment of cooperation for nursing care from a patient who used to refuse the care. Furthermore, a method of the training applied to the caregivers to feel relieved because of understanding how to interact a patient who responds emotional reacts, such as anger. Hereafter, we analyze the results of the evaluation deeply, and we are examining the effect of HCM for the professionals.
