• Note (General): type:text; 本研究の目的は、人生の終末期（エンドオブライフ:EOL）に本人の意思を尊重したよりよいケアを提供するために必要となるアセスメントツールを、デジタル環境に適応したクラウド上のアプリとして実現するための、要件定義とプロトタイプ実装である。これまで深堀らが開発し千葉県内の介護付き有料老人ホームで実装されているEOL ケアツールをベースに、居宅から施設への移動の際や、施設からの入院の際にタイムリーな情報共有が可能になるよう、ツールのクラウドアプリ化を目指す。本研究では、このために必要となる、EOLケアツールの利用手順の検討、システムの要求定義、機能要件の定義、プロトタイプシステムの実装、および研修用資料の開発を行った。 具体的には、下記の項目を実施した。 1. EOL ケアツールの要求定義：EOL ケアツールのチェックリストの内容が高齢者ケアの現場でどのように入力され、評価されるのか、また、蓄積したデータがどのように活用されるのかについてのサービスモデルを検討した。 2. 要件定義：1 の成果を元に、DX 版EOL ケアツールアプリが機能として持つべき要件を定義した。 3. プロトタイプシステムの実装：要件定義を元に、クラウドサービス上にシステムのプロトタイプを実装した。また、臨床業務を行うスタッフがシステムを学習するための研修用資料を開発した。 本プロトタイプの開発によって、深堀らが開発してきた紙ベースでのEOLケアツールでは困難であった施設間・法人間での情報共有と業務の効率化、またそれによって実現される、本人が望むケアをタイムリーに提供できる環境の概念実証(Proof of Concept)を実施することが可能となった。さらに、本システムに蓄積されたデータをよりよいケアの提供のための基礎的データとして活用するためのデータ基盤の可能性も示された。今後は、仮想的なユースケースによるシステムの評価及びデータ活用方法の検討を行う。 The purpose of this study is to develop an assessment tool for end-of-life (EOL) care as an application in the cloud adapted to the digital environment. The goal is to create a cloud application of the tools to enable timely information sharing during transfers from home to the facility and during hospitalization from the facility. This included a study of the procedures for using the EOL care tool, a definition of system requirements, functional requirements, implementation of a prototype system, and development of training materials necessary for this purpose. Specifically, the following items were undertaken. 1. requirement definition for the EOL Care Tool: A service model was studied for how the contents of the EOL Care Tool checklist would be entered and evaluated in elder care settings and how the accumulated data would be utilized. 2. requirement definition: Based on the results of 1, requirements that the DX version of the EOL Care Tool application should have as its functions were defined. 3. Prototype system implementation: A system prototype was implemented on the cloud service based on the requirement definition. In addition, training materials were developed for clinical staff to learn the system. Through the development of this prototype, functions for sharing information among facilities and corporations were implemented, which had been difficult with conventional paper-based EOL care tools. In addition, it is now possible to conduct a proof of concept study of an environment in which care desired by the patient can be provided promptly. Furthermore, the possibility of a data infrastructure for utilizing the data accumulated in this system as primary data for better care was also indicated. In the future, we will evaluate the system through a virtual use case and study how to utilize the data.
