• Note (General): type:text; 本研究は、予後の厳しい悪性胸膜中皮腫（malignant pleural mesothelioma; MPM）患者への新たな治療薬と個別化治療への臨床応用を目的としている。有効な臨床応用モデルの確立が必要であり、三次元的にin vitroでつくられた臓器であるオルガノイドを用いたMPMモデルの樹立に成功した。現在までに合計51ラインのMPMオルガノイド樹立を試みた。樹立は一時成功するも、多くの検体で継代が困難となることが判明した。現時点で９症例が安定して樹立できた。 樹立効率のさらなる改善のために、樹立に際して適するmediumの検討をおこなっている。他の中胚葉由来の生殖器系悪性腫瘍などのオルガノイド培養時のmediumでの培養も検討している。症例集積を継続しながら、同時に同一患者においての、治療前後の臨床検体でのオルガノイド樹立を継続する。これにより、治療に伴うMPMの分子進化を把握し、化学療法や免疫治療に対する耐性化機序などを同定できる可能性がある。 分子シグナル伝達経路を標的とした新たな治療探索のため、樹立したMPMオルガノイドでRNA-seqを用いてその分子異常の把握に努める。そして、MPMにおけるMETシグナル経路異常の生物学的意義を検証していく。現在樹立したオルガノイドを用いてゲノムシークエンスを進めており、それぞれのラインでMPMに特異的な遺伝子変異を認めることが判明した。RNAseqのデータから主成分解析PCAを行い、MPMは、腺癌, 扁平上皮癌, 小細胞肺癌, 正常肺胞, 気道上皮とは異なる分布を示した。Nicheの検討でYAP経路やFGF経路が増殖に関与することが予測されたため、現在RTKアレイなどでの解析を検討している。更に、もとの患者組織との相同性を確認するため、オルガノイドの免疫染色やXenograftモデルでの検証を行っている。 The aim of this study is to develop new therapeutic agents and clinical applications for individualized treatment for malignant pleural mesothelioma (MPM) patients with a severe prognosis. It is necessary to establish an effective clinical application model, and we succeeded in establishing an MPM model using an organoid, which is an organ made in vitro three-dimensionally. We have attempted to establish a total of 51 lines of MPM organoids. Although the establishment was temporarily successful, it was found that many specimens were difficult to subculture. At present, 9 cases have been stably established.In order to further improve the establishment efficiency, we are studying the suitable medium for establishment. We continue to establish organoids in clinical specimens before and after treatment in the same patient. In order to search for new treatments targeting molecular signaling pathways, we are going to strive to understand molecular abnormalities using RNA-seq.Then, we are going to verify the biological significance of MET signaling pathway abnormalities in MPM. We are currently proceeding with genome sequencing using the established organoids, and found that each line has MPM-specific gene mutations. Principal component analysis (PCA) was performed from RNAseq data, and MPM showed a different distribution from adenocarcinoma, squamous cell carcinoma, small cell lung cancer, normal alveoli, and airway epithelium. Furthermore, in order to confirm the homology with the original patient tissue, we are conducting immunostaining of organoids and verification with Xenograft models.
