Note (General): 胆道と膵は、発生学的に前腸内胚葉からほぼ同時期に派生し、またその組織構造や細胞形質も類似することが知られている。この胆道と膵との発生学的、解剖学的近似性、また細胞学的、組織学的類似性に着目し、胆道の疾患の病因と病態を膵との関連性で検討した。主な結果は以下に要約される。I)胆道と膵との炎症性疾患の一部(IgG4 関連硬化性疾患、アルコール性疾患など)は、しばしば合併し、共通の病理像を呈した。II)膵管癌と胆管癌は、その発癌機序(前癌病変を含む)、病態、生物学的態度に類似性がみられた。III)さらに胆管内乳頭状腫瘍と膵管内乳頭状腫瘍はその形態や形質が類似していた。今回の研究で、胆道疾患と膵疾患の成り立ちと病態に類似性があり、今回得られた成績は、今後の治療医学に応用可能と考えられた。
Biliary tract and pancreas derive from endoderm foregut almost same time, and both show similar tissue and cellular structures and phenotypes. With respect to these similarities, the etiology and pathogenesis of biliary tract diseases were examined. The data obtained in this study were summarized as follows. I)Some of inflammatory diseases of the biliary tract and pancreas such as IgG4-related scloerosing disease and alcoholic pancreatitis and biliary disease were frequently overlapped and showed similar pathological features. II)Pancreatic duct adenocarcinoma and biliary tract carcinoma including their pre-neoplastic lesions showed similar pathological and phenotypic features. III) Intraductal papillary tumor of the pancreas and biliary tract also showed similar features and phenotypes. In conclusion, the data obtained in this study based on the similarities between pancreas and biliary tract were applicable in the therapeutical approaches of the biliary tract diseases.
