• Note (General): コレステリルエステル転送蛋白(CETP)はHDLとアポB含有リポ蛋白との間でCEを転送する。1980年、我々が最初に報告した家族性CETP欠損症に著しい高HDL血症が認められたことより、CETPのHDL代謝への大きな影響が認識されてきた。今回の研究で、我々はイントロン14のスプライシング異常によるCETP欠損症の遺伝子診断法を確立し、ヘテロおよびホモ接合体を診断し、リポ蛋白異常を明らかにした。1.本症はわが国でのみ発見されている代謝異常であり,このCETP遺伝子変異によるCETP欠損症が同じ起源を持つ遺伝子変異によることが、ハプロタイプの検討より明かとなった。2,我々が開発した、PCR法により新しい制限酵素切断部位を作成する方法は条件設定が比較的容易であり、アイソト-プを使用せず短時間で結果が判定できるスクリ-ニングに適した方法であった。HDLーCが120mg/dl以上での異常遺伝子発現頻度は52%であり、100mg/dl以上では28%であった。HDLーCが120mg/dl以上の21例中14例にこの遺伝子異常が認められたが、残り7例は別の原因による高HDLーC血症であった。3,ヘテロ接合体のHDLは量的には僅かのCEの増加を示すのみであったが、脂質組成では明らかなCETP量の低下の影響が認められた。さらに、CETP量とIDLのECおよびEC/TG比との間に正の相関が認められ、LDL同様に動脈硬化を促進するIDLーCの増加とCETPとの関係が示唆された。従って、CETP欠損症ではCETPの量の程度により動脈硬化を促進するリポ蛋白が減少し、また、動脈硬化を予防するリポ蛋白が増加していると思われた。以上より、CETPの減少は血清リポ蛋白を抗動脈硬化的に変化させている。CETPが約半量のCETP欠損症のヘテロ接合体での冠動脈硬化症の頻度の検討が今後の課題である。; Cholesteryl ester transfer protein (CETP) can transfer cholesteryl ester from high density lipoprotein to apoB-containing lipoproteins. Our first report of familial CETP deficiency in 1980, revealed that the deficiency of CETP caused extremely high HDL cholesterol levels. Therefore, CETP plays an important role in HDL cholesterol metabolism. The CETP deficiency is caused by the mutation of intron 14 splice donor site of CETP gene, and the mutation of the CETP gene is common in CETP deficiency, resulted from the study of haplotype analysis.In this study, we developed a new methods about detecting the intron 14 splicing defect in CETP deficiency, and have searched the heterozygote of CETP deficiency. Allele frequency of the mutation was 8.0 % and 52.4 % in the groups of 100-119 mg/dl and equal or exceeded 120 mg/dl of HDL cholesterol. In the group of equal or exceeded 120 mg/dl of HDL cholesterol, 14 patients i 21 had the mutation of CETP gene, but other 7 patients had not. There was an increase of cholesteryl ester/triglyceride ratio in HDL in the heterozygous patients with CETP deficiency. Serum CETP concentration positively related to the intermediate density lipoprotein (IDL) cholesteryl ester and cholesteryl ester/triglyceride ratio. Thus, since the IDL cholesterol is atherogenic, the reduced IDL cholesterol in the heterozygotes may be anti-atherogenic.In conclusion, there was an anti-atherogenic change in the lipoprotein pattern in plasma obtained from the heterozygous patients with CETP deficiency.
