• Note (General): コレステロールエステル転送蛋白(CETP)は、アポB含有リポ蛋白(LpーB)へコレステリルエステル(CE)を転送する蛋白であり、動脈硬化の進展、退縮に重要と考えられているコレステロール逆転送系に関係している。我々はCETPと動脈硬化症との関係を検討し、以下の結果をえた。1、冠動脈造影所見より、冠動脈狭窄はHDLーC、HDLーC/Al比と負相関を示した。このHDLーCおよびHDLーC/Al比の変化はアポB値と有意の負の相関を示し、HDLーCの低下における、LpーBの増加の影響が示唆された。2、家族性高コレステロール血症患者(FH)における比重1.063以上分画のアポE/アポAl比は正常者より増加していた。アポE含有HDL(EーHDL)の増加がヘテロ接合体およびホモ接合体FHで認められ、コレステロール逆転送系の障害が示唆された。ヘテロFHのCETAは正常より高値であり、ホモFHでは正常の4倍の高値であった。また、CETAとHDLーC/Al比とは負の相関が認められた。3、CETP遺伝子はTaqI制限酵素により2種のRFLPを示した(A:9.0Kb/7.5Kb,5.3Kb/4.4Kb)。CETAの欠損している家族性高HDL血症患者のRFLPを検討した。完全欠損と考えられた発端者(SY)においてもCETP遺伝子は存在し、7.5Kbと4.4Kbのホモ接合であった。この家系内でのHDLーCの高値とRFLPのタイプとの間の関係は認められなかった。FHでのCETP遺伝子のRFLPが同様に検討された。特定のalleleとCETAとの間に有意の関係はみられなかったが、HDLーC/Al比とRFLPのパターンとの関係が認められる家系も存在し、HDLの変化に対するCETP遺伝子の関与が示唆された。以上より、CETPは高脂血症では増加しており、HDLーCの低下とLpーBのCEの増加に関与し、動脈硬化に促進的に働くと考えられた。CETAの調節には遺伝子レベルでの関与も示唆された。; Cholesteryl-ester transfer protein (CETP) transfers and exchanges cholesteryl esters between liporoteins. An increase of cholesteryl-ester transfer activities (CETA) may result in low levels of high-density-lipoprotein cholesterol (HDL-C). To investigate a role of CETP on progression or regression of atherosclerosis, CETA and HDL-C/apoAI ratio were examined in patients with familial hypercholesterolemia (FH) or coronary heart disease. Restriction fragment length polymorphisms (RFLP) of CETP gene were analyzed on FH or familial hyperalphalipoproteinemia with deficiency of CETA.Coronary atherosclerosis indeces which were estimated from the findings of selective coronary cine angiogram, showed a negative correlation with HDL-C levels and HDL-C/apoAI ratios. The decrease of HDL-C level was related to the increase of apoB-containing lipoprotiens. On plasma of FH, there were significantly high activities of cholesteryl ester transfer between HDL and other lipoproteins, and a negative correlation between CETA and HDL-C/apoAI ratio. Taq I RFLP of CETP gene was analyzed in patients with familial hyperalphalipoproteinemia or FH. A familial hyperalphalipoproteinemic patient with CETA deficiency showed a homozygosity in RFLP of CETP gene. The relationship between HDL-C/apoAI ration and RFLP patten of CETP gene was recognized in members of a FH family.These results indicated that CETA increased in hyperlipoproteinemia.
