【目的】肺癌に対する「体外肺切除」における、キトサンシートの気管支吻合部創傷治癒への効果と安全性について検証。【対象と方法】犬で体外肺切除モデルを作成し、気管支吻合部被覆を①対照群：被覆なし、②C群：キトサンシート被覆、③M群：肋間筋被覆、に分け比較。【結果】対照群1頭は気管支縫合不全、膿胸で死亡。他は全頭犠牲死まで合併症なく生存。気管支吻合部組織血流、気管支吻合部の炎症改善、胸部Xpでの肺水腫改善において、いずれもC群が最も良好。気管支吻合部外側の肉芽・血管新生はM群＞C群＞対照群。【結語】体外肺切除の気管支吻合において、キトサンシート被覆は安全に創傷治癒を促進し、肋間筋被覆よりも有用。
Aim: We investigated the efficacy and safety of chitosan sheet covering for bronchial anastomosis wound healing in "extracorporeal lung resection (ECLR)" for lung cancer. Materials and methods: ECLR model was prepared and applied to dogs in control group (no covering), C group (chitosan sheet covering), and M group (intercostal muscle covering). Bronchial anastomotic site healing statuses were compared among the groups. Results: One dog of the control group died due to bronchial fistula accompanying empyema. All other dogs survived with no complication. C group had the highest recovery in tissue blood flow of bronchial anastomotic site, inflammation improvement of anastomotic site, and lung edema improvement on chest X-ray films. Granulation and angiogenesis around the anastomotic site were promoted the most in M group followed by C group and then the control group. Conclusion: For bronchial anastomosis wound healing in ECLR, chitosan sheet covering is safe and promotes wound healing.
