トロンボモジュリン(TM)の一塩基多型(SNP)と造血幹細胞移植治療成績との関連を検討した結果、TM 発現が高いSNPを有するドナーでは急性GVHDの発現が低下した。放射線傷害の検討では、照射により血管内皮細胞上の組織因子および炎症性サイトカイン、接着蛋白のmRNA発現量は増加するが、TM発現量は減少した。HO-1は増加傾向を示したが有意な変動を示さなかった。照射前にTMを投与しておくと、TF発現量が低下した。以上の結果より、移植前のTM製剤の投与は、移植後の凝固活性化を抑制し移植関連合併症を改善する可能性が示された。
We revealed that the THBD gene single nucleotide variation rs3176123 in the donor side influences a risk for acute graft-versus-host disease (GVHD) and the subsequent mortality after unrelated bone marrow transplantation.　We also demonstrated that exposure of radiation on human umbilical vein endothelial cells resulted in significant increases in tissue factor (TF), IL-6, and ICAM-1 mRNA expressions. On the other hand, thrombomodulin (TM) mRNA expression was significantly decreased. Pretreatment of recombinant TM inhibited radiation-induced expression of TF. Recombinant TM would be effective for the prevention of transplant-related complications.
