心臓における交感神経刺激受容体焼灼の効果を検討するために心房細動におけるアブレーション前後での筋交感神経活動の評価を行った。心不全において、左心房内の焼灼をおこなうことにより慢性期の交感神経活動の低下がみられていた。このことは求心性の交感神経活動が低下し、結果として遠心性の交感神経活動である筋交感神経活動が低下したと考えられる。過剰な交感神経活動の亢進は心機能の低下につながるため、心房内の受容体焼灼は、心房細動を抑えるだけでなく有効な治療と考えられる。この結果は、心房細動のアブレーションが、付加的に交感神経活動の低下作用ももたらし心不全に対して効果的に働いている可能性が示唆さる。
In this project, I examined the effect of catheter ablation on muscle sympathetic nerve activity in patient with atrial fibrillation. Also I compared these effect between the patient with and without heart failure. Catheter ablation reduced muscle sympathetic nerve activity in the patient with heart failure, however, it did not change in the patient with out heart failure. Our result showed that efferent sympathetic nerve activity assessed by muscle sympathetic nerve activity is reduced by the reduction in augmented afferent sympatho-stimulating receptor in left atrium in heart failure patient. it is likely that afferent sympatho-stimulating might not be augmented in the patient without heart failure. these results indicate that catheter ablation in atrial fibrillation in heart failure might exert an additional effect on the reduction in sympathetic nerve activity, which contribute to the improvement in mortality.
