慢性腎臓病では早期発見が重要である。そのため、軽微な腎障害の病態を反映し、その早期診断、治療反応性、予後を反映するバイオマーカー確立は慢性腎臓病の予後改善にむけて重要な課題である。本研究では、慢性腎臓病においてこのような病態を反映するアミノ酸の同定を行った。生体内のアミノ酸は光学異性体であるD体とL体が存在する。本研究では光学異性体のアミノ酸を区別し、かつ網羅的に解析可能なキラルアミノ酸メタボローム解析を用いた。その結果、慢性腎臓病の早期診断、予後などと関連するアミノ酸を同定した。腎生検によらない慢性腎臓病の非侵襲的診断、治療、予後を反映するバイオマーカーの確立に資する研究を施行し得た。
The number of patients suffering from chronic kidney disease (CKD), which is a risk factor for end-stage kidney disease resulting in requirement for renal replacement therapy is increasing all over the world. To overcome CKD, the establishment of adequate clinico-pathological parameters and related biomarkers for diagnosis and/or prediction for prognoses is required. Recent advances in research have shed light on the chirality of amino acids. The two-dimensional (2D) HPLC technique reveals that free D-amino acids and their L-forms are quantified quantitatively. In this study, we aimed to examine the pathophysiology of kidney diseases depending on amino acids and related relevant biomarker candidates for chronic kidney disease. As a result, we determined biomarker candidates, which were associated with the progression of chronic kidney disease.
