Note (General): 本研究では呼吸循環器機能が障害される膠原病患者の運動中の心機能評価を非侵襲的心拍出量計を使用し、健常者の6分間歩行試験中の心機能と比較した。結果は、患者において心機能が安静時だけでなく運動時にも低下しており、肺機能との間に関連性があったが、心エコー検査で得られる評価と関連性がなかった。但し、心臓カテーテル検査で証明された肺高血圧症については心機能の運動に対する応答を低下させた。また縦断的研究として、運動中の心機能の経時的変化は肺機能の経時的変化と関連性があったが、歩行距離の変化は肺機能の変化とは関連性がなく、心拍数の代償を受けていた可能性が示唆された。
In this study, the cardiac function in connective tissue disease, which is known to impair cardiopulmonary function, was compared to that in healthy control during the six-minute walk test with a non-invasive impedance cardiograph device. The results showed the cardiac function in patients was impaired at rest and during walking, in relation to the pulmonary function but not to the cardiac parameters evaluated by transthoracic echocardiography. However, pulmonary hypertension detected by right-heart catheterization reduced the cardiac response during walking. The longitudinal study showed the cardiac function was changed according to the fluctuating pulmonary function, while the distance walked was not changed by the pulmonary function but might be compensated by heart rates.
