心房細動は高齢化に伴い、本邦でも増加している不整脈疾患であり、心不全、脳梗塞の原因となることが知られている。心房細動には頻脈と脈不整という二つの側面があり、頻脈による影響は過去に多く検討されているが、脈不整の立場からの検討は過去に行われていない。そこで、脈不整の程度を適宜調整可能なペーシング装置を用いて、脈不整がブタの心臓において血栓形成に寄与しているかどうかを検討することとした。全身麻酔下で、外科的にブタの左心房を２時間ペーシングした。ペーシング前後にそれぞれ採血を実施し、凝固能や炎症マーカーを評価した。不整ペーシング4匹、整ペーシング5匹の実験を終了し、現在各種マーカーの解析中である。
Atrial fibrillation (Afib) is an increasing arrhythmic disorder in the graying society like Japan, and it is known as a cause of heart failure and embolic stroke. Afib has two aspects, tachycardia and pulse irregularity, and the effect of tachycardia on heart failure or stroke has been well-investigated, while no reports regarding the effect of pulse irregularity have not been found. Therefore, we ought to investigate whether pulse irregularity played an vital role in thrombogenesis using porcine hearts.Under general anesthesia, porcine hearts were surgically paced for two hours. Blood samples were taken before and after pacing, and coagulation and inflammation markers were evaluated. We performed four cases of irregular pacing and five cases of regular pacing, and now several markers are being analyzed.
