手で触知する事が困難であろう小肺結節（肺癌等）や手による触知を使用できない胸腔鏡下手術において肺の局所切除を施行するためには病変を視覚化する必要であり、本研究の目的は、理想的（既存の方法より有効かつ安全）な方法、すなわち新しい肺マーキング法の確立であった。平成25年度研究では実験動物を使用し、光線力学診断用内視鏡システムとビタミンB2を利用した蛍光による経気管支鏡的マーキング法を優れた描出力、十分な描出時間、安全性を兼ね備えた理想的なマーキング法として確立した。平成26年度研究では、より臨床での状況を想定し、同じく実験動物を使用し、手技の改良および有用性・安全性の評価を行い良好な成績を得た。
When performing sublobar resection for small lung nodules, accurate identification of the location and extent of target lesion is essential. Especially in thoracoscopic surgery, we have to visualize the target without palpation. There are various marking procedures for small lung nodules, but each method has disadvantages and complications. The aim of this study was to invent an ideal lung marking method which was more accurate and more safe compared to previous ones.In 2014, we invented a novel less-invasive marking technique using this fluorescence technique using photodynamic diagnosis endoscope system and vitamin B2 in porcine experimental model. The fluorescence marking from this technique had sufficient visualization with enough time and substantial safety. In 2015, we modified and evaluated this technique with further clinical setting using another porcine experimental model. From the results, the prominent efficacy and safety of this new technique was demonstrated.
