NAFLD患者179人の臨床マーカーと肝病理像に関する横断的検討では、年齢・性・BMIで補正した重回帰分析の結果よりIRI、HOMA-R、トランスアミナーゼは各病理像(脂肪化、線維化、肝細胞変性、活動性)を、ヒアルロン酸は線維化と肝細胞変性を、高感度CRPは活動性を予知することが明らかとなった。連続肝生検NAFLD患者39人を平均3.0±2.1年間観察したところ、肝線維化は11人(28%)が悪化、12人(31%)が改善、16人(41%)が不変であった。年齢、性、BMIで補正した重回帰分析では、HbA_<1c>低下量のみが肝線維化進展の予測因子であった。以上より、血糖コントロールの改善が、NAFLD患者の肝病理改善を予知することが解明された。
