アセチルコリン受容体中のアセチルコリン結合部位のペプチドを,ラットに経皮的に投与することによって,重症筋無力症の免疫学的モデルを作成し,非脱分極性筋弛緩薬の作用を検討することが目的であった.しかし,非脱分極性筋弛緩薬の作用,特に末梢筋と呼吸筋への作用の違いについては,正常ラットでのデータが極端に不足していた.このため,平成13,14年度は,正常ラットでの末梢筋と呼吸筋の実験を主に行った.非脱分極性筋弛緩薬としては,ステロイド系のパンクロニウム,ベクロニウム,ロクロニウムを使用し,投与方法としては,持続投与のほかに単回投与を行った.正常ラットでのin vivoの前脛骨筋への筋弛緩薬の作用は,様々な報告があるものの,横隔膜への作用は,実験モデル作成が相当に困難であるため,検討された報告はなかった.我々は,胸骨切開法,固定法,止血法の改善,人工呼吸の工夫によって,横隔膜のin vivoでの実験モデルを平成14年度に作成することができた.これらの結果,横隔膜の非脱分極性筋弛緩薬のeffective doseは,前脛骨筋よりも大きくなり,この傾向は持続投与,単回投与共に同様であった.両筋のeffective doseの比(横隔膜のeffective dose/前脛骨筋のeffective dose)はパンクロニウム,ベクロニウム,ロクロニウム共に2以上であった.非脱分極性筋弛緩薬は,呼吸筋に対して抵抗性が大きく,末梢筋とは性質が異なることが判明した.現在,重症筋無力症ラットモデルを作成中である.
