Fractalkine/CX_3CL1(FKN)は活性化血管内皮細胞上に発現し、ケモカインとしての遊走活性と接着分子としての機能を合わせ持つユニークなケモカインである。そのレセプターであるCX_3CR1は細胞障害活性を有する単核球に発現する。我々は、FKNの全身性強皮症(SSc)の病態における関与について検討した。SScの皮膚や肺病変部の血管内皮細胞では、免疫組織染色にてFKNの発現が正常コントロールよりも亢進していた。血清と気管支肺胞洗浄液中のsoluble FKN(sFKN)をELISAにて測定したところ、SScで有意に上昇しており皮膚や肺病変の重症度および炎症所見と相関した。また、経時的なsFKNの変動を調べたところ、発症早期の病勢の強い時期に上昇が認められた。FKNのレセプターであるCX_3CR1は、フローサイトメトリーによる検討でSScの末梢血中の単球やT細胞で発現が上昇していた。以上の結果より、SScではFKNとCX_3CR1の発現が亢進しており、FKNとCX_3CR1を介した接着・遊走作用は細胞障害活性を有する細胞を罹患臓器に浸潤させ、SScにおける血管障害、炎症、組織障害やその後の修復過程による線維化などの病態に関与している可能性が示唆された。また、sFKNはSScの病勢や肺病変の血清学的なマーカーとなる可能性が示唆された。
