エコーと画像処理を用いて口腔機能改善のための介入方法の効果を評価する方法を確立することを目的とした。口腔機能の低下と関連する梨状窩の残留物は甲状軟骨を基準とした横断走査で観察が可能であり、経験を十分積んだ看護師による残留検知の感度は92.0%、特異度は71.9%と、先行研究と比べて向上していた。さらに、作成した残留物を自動で着色する画像処理プログラムは観察者の判断を支援できる可能性が示された。今後は嚥下体操などの口腔機能の改善のための介入を行う対象に画像処理を用いたエコーによる咽頭残留の観察を行い、残留物が減少するかどうか確認する有効性検証を行う必要がある。
This study aimed to establish a method to evaluate the effectiveness of intervention methods to improve oral function using ultrasound. Residues in the pyriform sinus, which was associated with deterioration of oral function, could be observed on a cross-sectional scan above the thyroid cartilage, and the improved sensitivity and specificity of residual detection by an experienced nurse were 92.0% and 71.9%, respectively. Furthermore, it was shown that an image processing program that automatically colored the residues could assist the observer's evaluation. Future studies are required to evaluate the reduction of the pharyngeal residue as an improvement of oral function by ultrasound with image processing.
