本研究は,ベンチマーキング作成に必要な急性期病院における転倒予測アセスメントツールを検討することを目的とした3年間にわたるものである。(1)研究者らが開発したアセスメントツールを使用し,実際の転倒者について検討した。我々が開発した急性期バージョンのアセスメントツールの項目は(1)転倒経験,(2)認知レベル,(3)排泄介助,(4)椅子からの立ち上がり,(5)移動レベル,(6)内服薬(抗てんかん薬,精神安定剤,睡眠剤),(7)直感の7項目である。この新しいツールを2つの特定機能病院で3ヶ月使用したところカットオフ値2点で感度66.7%,特異度32.7%であった。しかし,相対危険比は3以上であり,非転倒者と転倒者には有意差がみられた。さらに(2)石川県下の急性期病院6施設での内部ベンチマーキングとしての転倒率・傷害を伴う転倒率,傷害の指標などを調査した。転倒率は2006年度1.11〜3.79,2007年度1.57〜3.50であり,傷害を伴った転倒率は,2006年度0.03〜0.69,2007年度0.23〜0.87であった。以上から開発したアセスメントツールは,ほぼ有効性があるものの,特異度が低いのは入院時は治療前で転倒のローリスク者を発見できにくいことが示唆された。また,内部ベンチマーキングは米国の報告より低い値を示した。この要因は病院間での転倒の定義などのちがいによると考えられ,さらに継続的な検討の必要性が示唆された。
The purpose of this study is development of the fall risk assessment tool in patients of acute care hospitals which is necessary for benchmarking. First, we identified the definition of benchmarking, and then fall risk assessment tool was examined for the falling experience of the hospitalized patients in acute care. Secondly, we developed the fall risk assessment tool for the acute care term version as based upon the effort for the prevention against falls at Providence Hospital in U.S.A. Then we used these tool for 3 months at 2 acute care hospitals in Japan. Using two points as the cutoff for high risk of falls, sensibility was 66.7%, and specificity was 34%. And the rate of fall in 6 acute care hospitals was 1.11-3.91 for inward benchmarking, and specified rate of fall of patients with injury was 0.03-0.45. These results were evaluated as lower rate than reported in U.S.A. The difference of definition of fall was considered to cause these situations.
