アトピー性皮膚炎(AD)患者56例と健常人30例より末梢血を採取し、同一血清中のTh2ケモカインおよびTh1ケモカイン濃度をモノクローナル抗体(R&D社)を用いたELISA法によって測定した。AD患者ではTh2ケモカインであるTARC、MDCならびにTh1ケモカインであるMigの血清中濃度が対照と比べて有意に上昇していた(96.36±38.12 VS 0.18±0.06ng/ml、3.27±0.67 VS 0.20±0.04ng/ml、38.69±1.23 VS 24.11±1.14pg/ml)。AD患者の詳細なデータベースをもとに、重症度、各検査異常、合併症、および治療などと各ケモカイン測定値との関連について解析を行ったところ、TARC、MDC濃度は、重症例では中等症、軽症例よりも高値を示し、各患者末梢血中の総IgE濃度や好酸球数と有意に相関していた。また、TARCおよびMDC濃度とMig濃度の間には有意な正の相関を認めた。以上の結果よりTh2とTh1ケモカインの両者がADの病態形成に何らかの役割を果たしている可能性が示唆され、TARCおよびMDC濃度はADの重症度を反映する血清学的なマーカーとなりうることが示された。次に、AD病変部皮膚でのTARC、MDCおよびMigの発現を免疫組織染色によって検討した。その結果、AD病変部の表皮基底層においてTARCの発現増強が観察され、真皮血管周囲にMDC陽性細胞が多数認められた。
