【研究の背景と目的】血管奇形は多種多様な病態をとり、治療法は確立していない。したがって、将来的には遺伝子治療も視野にいれた新しい治療法の開発が必要である。本研究では家族性静脈奇形において異常が報告されたTIE2遺伝子及びそのリガンドであるAngiopoietinに注目し、血管奇形組織を用いて、その遺伝子変異と発現を解析した。【対象と方法】静脈奇形20例、動静脈奇形6例を対象とした。コントロールとして正常血管組織5例を用いた。(1)DNA、RNAの抽出:パラフィン切片または凍結切片からマイクロダイセクションにより病変組織と隣接する正常組織を分離し、フェノール/クロロホルム法によりDNAを、AGPC法によりRNAを描出し、cDNAを作成した。(2)LOHの解析:TIE2遺伝子の存在する9p21領域のマイクロサテライトマーカーを用いて、DNAをPCR法にて増幅し、病変部DNAのバンドの一方のバンドが正常組織と比較して明らかに減弱している症例をLOHと判定した。(3)RT-PCR法:病変組織におけるTIE2及びAngiopoietinのmRNAレベルにおける発現をRT-PCR法にて定量化した。(4)免疫組織化学的検討:免疫組織化学的にTIE2遺伝子の発現様式の検討を行った。【結果】LOH解析では、静脈奇形3例でLOHを認めた。RT-PCR法による解析では、静脈奇形8例、動静脈奇形2例でAingiopoietin2の発現の増強を認めた。免疫組織化学的検討では、TIE2の発現の増強は認められなかった。【考察】ヒト組織を用いた血管奇形の検討の結果、TIE2遺伝子が静脈奇形の発症に関与している可能性が示唆された。また、TIE2のリガンドであるAngiopoietin2が血管奇形および血管系腫瘍の発症および進展に重要な役割を果たしている可能性が示唆された。
