ヒアルロン酸とポリリジンからなるゲル膜と除細動リードからなるゲルパッドを作製した。成豚12頭を2群(CO群,CC群)各々6頭に分け検討した。CO群は開胸,CC群は閉胸状態で除細動可能かを検討した。ゲルパッドを全身麻酔下に胸骨正中切開して成豚の右,左房に留置し,Af誘発用ペーシングワイヤーを肺静脈に留置した。肺静脈をburstpacingし,Afを誘発した。その後,除細動を行い,0.5Jずつ除細動可能なエネルギーまで出力を上げていった。CO群:これを一頭につき5回施行した。CC群:同一出力で2回連続除細動可能であったエネルギーを除細動閾値とし,留置当日,閉胸状態で第1,3,5,7病日に行い除細動閾値とリード抵抗値を測定した。第10病日に除細動ワイヤーを抜去した。ゲルは任意期間留置ののち,分解性,消失性について肉眼的,病理学的評価を行った。除細動に要した全体的な平均エネルギー,リードインピーダンスは各々CO:CC=0.65±0.23:1.67±1.00J,97.6±5.52:75.9±13.3Ωであった。除細動リード抜去時の合併症は認められなかった。留置後1ケ月後のゲル分解性の肉眼的,病理学的評価では,ゲルパッドは菲薄しており,周囲組織との著名な炎症所見はみられなかった。開発したゲルパッドを用いた除細動治療は,心房細動に対して低出力による除細動が可能であり,また,ゲルパッドは生体内で分解され,リード抜去時の合併症を認めないことから従来法にかわる周術期不整脈治療法の一つとなりうることが示唆された。しかし,臨床応用にはゲルパッドに関する毒性試験や疼痛に関するさらなる評価を要すると考えられる。本研究は術後心房細動における新たな治療法の可能性を示した価値ある研究であり以下の論文,学会にて成果を発表した。
