● 研究目的リトドリン塩酸塩の添付文書には、新生児低血糖症が重要な基本的注意及び副作用として記載されているが、頻度などの詳細は不明である。そこで、リトドリン母体投与後の新生児低血糖症発現状況について調査し、リトドリン投与量・投与期間・投与中止後から分娩までの期間などとの関連性を検討した。● 研究方法母体にリトドリン持続点滴投与を受け投与中止後1週間以内に分娩となった新生児を対象とし、電子カルテを用いて調査を行った。児は正期産(37週以降)・後期早産(36週以降に限る)の正常新生児とし、出生時体重2000g未満の例は出生直後から点滴開始となるため対象から除外した。● 研究成果2013年8月～2016年7月の3年間で調査を行った。リトドリン投与群の新生児低血糖症発現率は61.5%、非投与群の低血糖症発現率は8.2%であり、母体リトドリン投与により新生児低血糖症の発現率が明らかに高くなった。リトドリン投与群の新生児低血糖症発現群における投与中止後から分娩までの期間は平均4.3時間(標準偏差7.61)、リトドリン投与群の低血糖症非発現群では平均37.7時間(標準偏差46.00)であり、有意な差が認められた。低血糖症発現群の67.2%が投与中止後から分娩まで3時間以内であった。リトドリン投与中止後から分娩までの期間が新生児低血糖症発現に関与していることから、計画的にリトドリン持続点滴投与を中止することにより新生児低血糖症を回避できる可能性を示唆した。また、リトドリン投与中止後から分娩までの期間により新生児低血糖症発現のリスクが予見でき、確実な予防対策を講じることができると考えられた。
