フローサイトメトリー(FCM)法で当院血液内科受診患者を対象に、血液がんの臨床経過とA・B・I抗原の発現量や蛍光強度を経時的に解析した。まず、スクリーニング検査として血液がんと診断された患者21例のA・B・I抗原の発現量や蛍光強度を測定した。A抗原を持つ患者13例中4例で80%未満の発現率であった。B抗原を持ち患者8例中4例で80%未満の発現率であった。I抗原はこれまでの検討で78%以下に血液疾患による減弱の可能性が高いことを報告した。よって、スクリーニング時点でI抗原の発現率が78%以上の場合は除外し、かつ当院で経過観察可能であった7例の発現率の推移を半年～1年間確認した。まず高齢による未治療2例は、定期的に輸血を実施しつつも、I抗原量は60%～70%を推移した。A、B抗原についても経時的観察でほぼ変化は認めなかった。次に、外来による経過観察中2例に関しては、A・B・I抗原は大きな変動は認めなかった。残り3例はアザチジンによるMDSの治療中はほぼ変化は認めなかったが、病勢憎悪にA・B・I抗原の低下を認めた。その内1例のO型の急性転化の例で寛解導入療法前後のI抗原の発現率が78%から95%へ変化し、強い抗がん剤治療と輸血により改善したと考えられる。以上より、病勢が悪化の時は抗原量の発現が低下する傾向にあるが、状態が安定している時は一定の幅で推移すると考えられる。今後、大多数での検討と長期にわたる経過観察により発現動態を検討が必要と考えられる。
