1.重症・無自覚低血糖の実態北陸小児糖尿病サマーキャンプに参加した患者133名(男58名、女85名)のキヤンプ記録より調査した。その結果、インスリン注射の発達とともにインスリンに関する課題は分類された。1期は精製インスリンによる1日1回であり、注射部位の筋肉組織の萎縮であり美容上の問題や吸収の問題であった。2期はヒトインスリンによるもので1日、2回が多くなった。注射手技の徹底と夜中および明け方の低血糖対策であった。さらに3期は3-4回となり、学校での注射に関すると頻回な低血糖への対策であった。また重症・無自覚低血糖経験は、1期では甲状腺異常のある子など特定人に限定されており、罹病期間が長くなった思春期以降に体験者が多くなっていたが、2期から3期では、注射導入して短期間から始まり、血糖を低くしようとする親の子にその頻度が高い傾向であった。それそれの頻度は1期が16.9%(男の発症率6.7%、女27.5%)2期が18.9%(男5.3%女25.0%)3期が34.5%。(男30.0%、女368%)であった。2.予防指針7歳女児の糖尿病発症から2年間の学校での血糖の動きと対策についての毎日の記録およびサマーキヤンプでの自由討論からから低血糖の自覚から無自覚になる軌跡を辿った。また子供のセルフケアを親がどうみて周囲への周知をどのように考えているかについての調査紙による調査も行った。その結果、重症・無自覚低血糖は糖尿病発症初期1-2年間に、子供が感じているからだの違和感を(アウエアネス)軌跡として明確にして、違和感と理由と対策を教え、これまで以上に細かく低血糖対策をたてる、そのために親に徹底した低血糖対策教育が必要であることが明らかになった。教育内容には、食物選択の知識の充実が急務であることが示唆された。

The population of female severe hypoglycemia is larger than that of male. There were 133 (male 58, female 85) patients in Hokuriku-children IDDM summer camp, 34.5% of whom are experience no subjective symptom and severe hypoglycemia.2) Guideline of prevention in subjective symptom and severe hypoglycemia in patients with IDDM Over the following 2 years blood glucose level and subjective symptoms, and awareness, low BG detection Judgment to treat, self treatment behavior, severe hypoglycemia coping patients reported severe hypoglycemia in school. Teachers and parents educated blood glucose level and cope with a hypoglycemia. But they did not educate awareness, low BG detection, judgment to treat, self treatment behavior.
