慢性ウイルス肝炎からの肝発癌に関与するウイルス側、宿主側要因を明らかにする目的で、マウスモデルを用いて検討した。B型肝炎ウイルストランスジェニックマウスに、同系統の野生型マウスから骨髄細胞、脾細胞を移植することによって慢性肝炎モデルを作成した。肝炎の経過を観察すると、約17カ月後に肝細胞癌が発症した(J.Exp.Med.188:341,1998)。1.ウイルス側要因の検討として、肝組織におけるウイルス遺伝子の発現をRNA、タンパクレベルで検討すると、前癌状態、非癌組織における発現はやや低いレベルながら持続していた。癌組織におけるウイルス遺伝子の発現レベルには多様性を認め、ウイルス遺伝子または発現調節因子の変異が示唆された。2.宿主側要因の検討として、移植する脾細胞をCD4またはCD8陽性Tリンパ球、Bリンパ球、単球の各分画に分離して役割を検討したところ、主にCD8陽性Tリンパ球が慢性肝炎の増悪および遷延化に作用していた。そこで臨床応用を念頭に、CD8陽性Tリンパ球の細胞障害作用を抑制する目的で、エフェクター分子であるFasリガンドに対する中和抗体を投与したところ、慢性肝炎が抑制され肝発癌が著明に減少した。これより、肝発癌過程には以上のウイルス側、宿主側要因の関与が示唆された。
