• Note (General): 金沢大学医学部附属病院; 本研究費により平成14年度までに明らかとなった成果は以下の4点である。1.子宮内膜癌の約40%にhMLH1プロモーターのメチル化されている。2.hMLH1のメチル化に伴い蛋白発現が低下する。3.子宮内膜癌に近接する正常子宮内膜においてすでにhMLH1はメチル化される。4.子宮内膜癌のみならず正常内膜でもhMLH1のメチル化に伴う遺伝子不安定性を認める。以上の成果を踏まえ、15年度に明らかとなった成果を以下に示す。1.hMLH1プロモーターのメチル化質的解析メチル化解析法として一般的に用いられているのはMethylation specific PCR(MSP)と呼ばれる定性的な解析法であるが、今回hMLH1プロモーターの質的なメチル化解析を目的として用いた方法がBisulfite sequencingであり、プロモーター領域約700bpをsequencingすることによってメチル化の領域と頻度を直接確認した。その結果、子宮内膜癌症例56例のうち、16例(29%)がプロモーターのほぼ全領域がメチル化され、14(25%)が部分的なメチル化、26(46%)がメチル化されていない症例であった。2.PTEN mutationとの関連癌抑制遺伝子PTENは、子宮内膜癌の早期に変異を受けることが報告されている。同様に発癌の早期に認められるhMLH1のメチル化との関連を調べた結果、PTEN mutationは子宮内膜癌の約40%に認められ、特にPTEN frameshift mutationはhMLH1プロモーター全体がメチル化された症例に多く、有意な相関を認めた。3.Microdissection(Laser capture microdissection : LCM)本研究では癌病変と正常組織、前癌病変などを顕微鏡下に厳密に区別するため、LCMを用いている。これにより得られた極微量のDNAよりメチル化解析を行うことはこれまで技術的に困難であったが、我々はPCRとメチル化感受性制限酵素を組み合わせることによりこの問題を解決した。その結果、子宮内膜癌の前癌病変である子宮内膜増殖症でのメチル化頻度は、癌とほぼ同等であることを確認しており、現在症例を追加して検討を進めている。以上の結果を踏まえ、発癌の初期イベントに注目した子宮内膜癌発癌機構の解明と、スクリーニングにおける発癌リスクの高い症例を識別する臨床応用について、今後研究をさらに進める予定である。; The results previously cleared in this study are four points as follows ;1.The hyperinethylation of hMLH1 promoter is found about 40% of endometrial cancers.2.Protein expression is decreased with hyperinethylation of hMLH1 promoter.3.The hypermethylation is already found in normal endoinetrium adjacent to endometrial cacers.4.Genetic instability is also found both in cancer and normal endometrium with hypermethylation of hMLH1.With the above-mentioned results, the followings are cleared in 2003.1.Quantitative methylation analysis of hMLH1 promoterMethylation specific PCR(MSP)was used as qualitative analysis, however, quantitative analysis for hMLH1 promoter hypermetylation is required.For this purpose, we used bisulfite sequencing to investigate metylation status within 700-bp promoter area of hMLH1.With the 56 endometrial cancer cases, 16 cases (29%) are fully methylated in promoter, 14 cases(25%) are partially methylated, and 14 cases (46%) are non-methylated.2.
