Note (General): 金沢大学医学系研究科
30%エタノールの血管壁への影響、安全性について、急性期および亜急性期の成犬の腎動脈の変化を観察した結果、腎動脈撮影上は、いずれの撮影でも血管れん縮は全く起きていなかった。さらに腎動脈撮影直後と3週間後に摘出した腎動脈の病理組織でも血管内皮の損傷を含めた血管壁の変化は、全く認められず、30%エタノールは安全であることが判明した。EVAL(ethylene vinyl alcohol)の血管壁への影響と塞栓効果の観察でも、炎症細胞の浸潤が軽度認められたが、弾性板の断裂はなく、EVALの断片化もほとんど認められなかった。また、血管壁外へのEVALの露出は全く認められず、再開通も認められなかった。つまり、安全性に問題なく、動脈瘤の塞栓物質として適しているということが判明した。成犬の頸動脈にside wall typeの動脈瘤を作成し、マイクロカテーテルを動脈瘤に留置し、バルーン拡張型のステンを用いて動脈瘤の塞栓を行い、急性期、亜急性期の血管撮影を観察した結果、動脈瘤の造影は認められず、塞栓効果は完全であった。病理学的にも炎症細胞の浸潤が軽度認められたが、弾性板の断裂はなく、EVALの断片化もほとんど認められなかった。しかし、脳内の血管を観察すると、EVALが認められることがあり、完全にはEVALの遠位への塞栓を防ぎきれなかった。今後はEVALの重合時間を変化させて遠位血管への塞栓を防ぐ研究を行いたい。
We investigated the usefulness of aneurysmal embolization using liquid material and balloon-expandable stent. First, we studied the safety of liquid material consisted of ethylene vinyl alcohol and ethanol. Then, it was proved that the liquid material was very safe and easy to use. Using the animal model of the side-wall aneurysm, aneurysmal embolization using liquid material and balloon-expandable stent. Then, it was proved that the embolization we used was safe and useful, however, some small infraction was seen.
