全身性強皮症患者のうち、皮膚硬化が前腕の皮膚までにとどまるlSSc(limited cutaneous systemic sclerosis)患者では、健常人と比較して血中レプチン値は有意に高値である。また、皮膚硬化が前腕を越えて近位の皮膚にまで認められるdSSc(diffuse cutaneous systemic sclerosis)患者血清中のレプチン値は健常人と有意差を認めない。しかし、皮膚硬化と、皮膚硬化の指標であるTSS(total skin thickness score)とは負の相関がみられ、皮膚硬化が高度であるほどレプチン値は低いことが明らかになっている。そこで、FACSを用いてCD4+、CD19+、CD8+T細胞表面のレプチンレセプターを解析した。全身性強皮症患者(dSSc, lSSc)では、健常人と比較してT細胞表面のレセプターが少ないという一定の傾向を認めていない。検体数を増やして解析する必要がある。さらに、ほかの自己免疫疾患でも解析することが望ましい。また、全身性強皮症患者および健常人の末梢血T細胞を分離培養した。培養時、種々の濃度のレプチンを添加し、培養上清中のIL-4,IL-6,TGFβ、IFNγ、TNFα値を測定した。これまでのところ、Th1,およびTh2に関して、全身性強皮症患者(dSSc, lSSc)と健常人の間に優位な差を認めていない。今後検体数を増やして検討する必要性が残る。
